Welcome to the Lakeside High School 504 Newsletter! If you received an email regarding your student's 504 status, then you are in the right spot! Continue to scroll down to find the appointment request calendar, forms, and notices.
Parent forms need to be completed, scanned, and returned via email at least 5 days before your 504 meeting.
If you do not have printing and scanning capabilities, please notify us and we will prepare paper documents for you to pick up at the school.

Federal Law
Section 504 of the Rehabilitation Act of 1973, is a federal civil rights law that prevents intentional or unintentional discrimination against persons with disabilities. This legislation protects the civil rights of disabled persons.
Section 504 states: “No otherwise qualified individual with a disability in the US, as defined in section 706(8) of this title, shall, solely by reason of her or his disability, be excluded from the participation in, be denied the benefits of , or be subjected to discrimination under any program or activity receiving Federal financial assistance.”[42 U.S.C. § 794]
504 Highlights!
- Section 504 is NOT designed to directly improve grades, raise test scores, or reduce homework responsibilities. Section 504 provides EQUAL ACCESS to educational programs and services.
- Accommodations are intended to “level the playing field” and give the student an equal opportunity to participate in the general education program.
- Accommodations DO NOT reduce grade level or course expectations.
- The Section 504 Accommodation Plan is reviewed at least once a year.
- A physician’s medical diagnosis should be considered as valuable information, but a medical diagnosis alone does not result in 504 eligibility.
- Determinations of eligibility, accommodations, and services are made by the 504 Team.
- Anyone can refer a child for evaluation under Section 504; for example, Parents/guardians or Teachers can make a 504 referral.
- 504 plans should be reviewed once per a year.
504 Appeal Process
- Student's full name.
- Address of student's residence.
- Name of school your student attends.
- In detail, please describe the concern or issue you would like to appeal.
- Requested reasons for review.
- The proposed solution to the issue.
- Name and contact information of the person submitting the appeal.
