Complete the Middle College registration form
The form asks for contact information for the student and parents/guardians, emergency contacts, and basic health information. There is also a section that explains our expectations which needs to be electronically signed by both students and parents/guardians, and the FERPA section will allows MC personnel to communicate with the student, parents/guardians, home school, and college faculty.
Complete the Cañada College Application
All students must complete the college application. Completing the college application will generate your college ID number (G#). You will need this ID number for everything at the college. A completed college application will also generate a college email address. This is how you will receive all your college correspondence. You will first have to create an OpenCCC account; then you can complete the Cañada College application. Check the email address you used to create the account for confirmation from OpenCCC. You'll then receive an email from Cañada Admissions with your G# (college ID number).
When you fill out the college application, please choose concurrent enrollment on the enrollment information page.
How to complete the college application.
Send us a writing sample
All applicants must submit a typed 1-2 page response, double spaced. The purpose of this exercise is two-fold: to provide a writing sample so we can see your skill level, and to get to know you better.
Topic: Successful Middle College students are independent, motivated, hard workers. They don’t give up when confronted with difficulties, they ask for help when needed, and they manage their time well.
Prompt: Describe a time when you have shown resilience, persistence, maturity, and self-direction.
Send the response to canadamiddlecollege@smccd.edu or fax to 650-306-3128. Be sure the student's name is on the response.
Recommendation forms
Students need recommendation forms from two current teachers and the guidance counselor.

Teachers and counselors can return the forms via the SUHSD intraoffice mail or fax them to 650-306-3128.
Applicants not currently enrolled in a SUHSD school must also provide the most recent copy of their high school transcript and attendance report. Please fax these documents to 650-306-3128 or email to canadamiddlecollege@smccd.edu.
Once all components are complete, intake interviews will be scheduled with each student and her/his parent or guardian. Middle College faculty will contact applicants to schedule the interview.
Middle College is a program of all five Sequoia Union High School District comprehensive high schools; applicants must meet any and all regulations pertaining to enrollment in Carlmont, East Palo Alto Academy, Menlo-Atherton, Sequoia, and Woodside High Schools.
