Health
Student Medication:
To enable us to provide safe care for your child while at school, please submit the appropriate attached authorization for medication form(s). The Allergy Plan is used for anaphylaxis and specifically if epinephrine is prescribed. Please provide the medication and/or supplies necessary in a pharmacy-labeled container. As a reminder, students are not allowed to bring any type of prescribed or over-the-counter medication to school. Please refer to the PUSD Medication guidelines for questions. https://www.powayusd.com/apps/pages/health-services
If your learner has a documented health condition, It is helpful to know if there is or isn’t medication prescribed or if medication is at home if needed.
If we do not receive medication and necessary medication authorization forms, it will be our understanding that you do not want this service provided during the school day when your child is on campus. Please let me know if you have any questions or concerns.
Some helpful hints:
Medication authorization forms are valid for one school year term only. A new medication authorization form must be signed by a doctor and parent every school year term.
Confirm the prescribed medication details:
Confirm that you have the correct prescribed medication.
E.g. Benadryl (DIPHENHYDRAMINE) is different from Zyrtec (CETIRIZINE).
Confirm that you have the precise dosage cup/dropper/syringe for the prescribed dosage.
Confirm the medication has not expired.
Medication Forms:
If your child requires medication while on campus, you may submit PUSD medication authorization forms and prescribed medication at this time. Forms must be completed and signed by a CA physician and parents. For additional information on student medication and medication administration, please visit the PUSD Health Services webpage.
Please use appropriate MEDICATION FORMS:
Life Threatening Allergy Plan (used for anaphylaxis, must be used if Epinephrine is prescribed)
Authorization for Medication (used for asthma inhalers, OTC and prescribed medications)
Seizure Diastat (if Nayzilam is prescribed, please email for appropriate forms)
Food & Nutrition:
Students are welcome to enter the campus (at the side gate only) for breakfast beginning at 8:20am. At 8:30am breakfast service ends and students may continue to eat or join Connect Academy for class by 8:43am. To ensure the safety of our campus, all gates will close at 8:45am.
Food and Nutrition Updates and Reminders:
- School breakfast and school lunch are FREE for all enrolled students every school day. Meals must still be entered into the cafeteria computer by individual student accounts.
- Even though meals are free, families are still highly encouraged to complete the application for free and reduced-price meal eligibility. School site funding depends in part on the number of students eligible for free/reduced-price meals. Families that qualify for free/reduced-price meals may qualify for other benefits, as well. The meal application is available online at MySchoolApps.com.
- Printer-friendly PDF menus are available on the PUSD Food and Nutrition webpage. At the top of the page, click or hover over School Menus + and then Printable PDF Menus.
- Interactive, web-based menus containing nutrient and allergen details are available at MySchoolMenus.com, as well as via links from the F&N webpage.
A volunteer is described as someone supporting a school site on a regular and ongoing basis
Yearly Volunteer Requirements:
Submit a Level 1 application each school year with a valid, legal identification.
Sign the Code of Conduct and Code of Confidentiality each year.
Submit TB test results. Tests are good only for four years from the date of the test. *this does not transfer between school sites*
Staff must see and sign off that they have accurately seen a valid, legal ID belonging to the volunteer and verify that the information on the application matches.
