Medication Pick Up
It is important that your child's medication in the health room be picked up on or before the last day of school. Medication must be picked up by a parent, guardian, or adult designee. Medication that is not picked up by the end of the last day of school will be destroyed per DHEC's guidelines.
Medication/Medical Treatment/ Medical Procedure for Next School Year
Any consent forms for any of the above during the school year become null and void at the end of the school year. New signed consent forms will be required as necessary for each school year. Some medication consent forms can be downloaded from the district site. Go to Departments, School Health Services, Health Room Forms, and choose Medication Consent Form. This form is mainly used for routine daily medication, non routine or as needed medication, and conditions like asthma. There are other consent forms that are more specific for conditions like severe allergic reactions that require EpiPen (Epinephrine) and emergency seizure medication forms. These are not on the website. All of the forms can be obtained from your school nurse.
S.C. Immunization Certificate
It is required that all students have a compliant immunization certificate to attend school. If your child receives immunizations during the summer, please request a new immunization certificate for school. You may provide the certificate to the school during the summer. Once school begins and it is found that your child is not compliant, you will be given 30 days to comply or risk your child being excluded from school until compliance is met.
Contact Bobbie Boulware, School Nurse at 476-4009 for any question concerning any of the above.
Cafeteria Reminders
If your child eats lunch in the cafeteria, please account for $2.75 for each day your child eats with us.
Memorial Day/Last Two Days of School
Schools and offices in School District Five of Lexington & Richland Counties will be closed Monday, May 27, 2019 in observance of Memorial Day.
Wednesday, June 5 and Thursday, June 6 are scheduled as half days for students. Students will be dismissed at 10:45 am.
Media Center News
Is it possible that we are down to our last few weeks of school? It seems like we just began not too long ago, doesn’t it?
However, it is that time…time to start gathering the school’s library books and taking account of students with lost or damage fees.
All books are to be returned on or before Friday, May 24th.
If your child has been receiving notices for overdue books, please help him or her look for the book.
Unfortunately, records of books not returned or paid for will follow your child as he or she moves through the grades and/or changes schools.
If you pay for a book and it is found over the summer, you may return the book to the school office and a credit will be given – if the condition of the book is as it was when checked out by the student.
Or if you choose, we can issue a credit your child can use at the next book fair.
Thank you for helping us!

As a parent of a Leader in Me student, we would like to ask for a few minutes of your time to take this short survey. It should only take about 6 or 7 minutes and will give us valuable information on how we can continue to help our students become Leaders and Learners for Life. Surveys should be completed by June 6th. We appreciate your time!
