VOLUNTEER CLEARANCES

you must complete and turn in the following volunteer clearances to the school or district office:
Volunteer Registration Form
Child Abuse History Clearance
Criminal Records Check
Act 24 – Arrest/Conviction Report
Federal Criminal History Record
Tuberculin Test Results
Mandated Reporter Training
Clearance application packets, which include directions for completing the forms, are available in the school office. Please be aware that some of the above clearances may take several weeks to be completed.
IMPORTANT NOTES
1. Approval of an Educational Trip
If your child will be absent from school due to being out of town one day or several days (Farm Show, family trip, etc.), a Request for Approval Of An Educational Trip Form must be completed before the absence, for the missed days to be counted as an excused absence. Written notes for an out of town absence are unexcused.
2. Notes for Student Absences
Students who have been absent from school, must bring an excuse note, stating the reason and date of absence, within 3 days of their return to school or the absence will be marked unexcused.
3. Names on Outerwear
Now that the colder weather has arrived, we often have “leftover” coats, gloves and hats brought to our “lost and found” box. Please consider marking your child’s name on all outerwear so that lost items may be returned as soon as possible. If you child is missing a coat or gloves, you may call the school office to see if your item has been turned in.
