Do I Qualify? - ACP - Universal Service Administrative Company
A Representative from Excess Telecom will be at our Open House on September 28th from 4:40-6:00 PM to enroll families who qualify for a tablet with a one-time activation fee of $11. The tablet will have service via the T-Mobile network for 1 year and then families will have to re-certify. The cost for re-certification is not know at this time.

Hello everyone!
My Name is Stephany Rodriguez Moreno. I have currently taken the role of the Secondary Counselor and I am very excited to be working with all Center Consolidated School District students, staff, and teachers. I am very excited to be working in CCSD as I was once a student here. A little bit about myself I attended Adams State University and received my Bachelors of Arts in Interdisciplinary Studies; an emphasis in Psychology, Sociology, and a minor in Spanish. I am currently in my second year of my Masters in School Counseling and Clinical Mental Health Counseling. I love being outdoors and working out. In my free time, you can find me hanging out with my family or friends. I am bilingual both in English and Spanish. My favorite quote is: Education is the only thing that no one can take from you. - B. B. King. To add to this quote I am a first-generation migrant Latina student. I am located in room s223, so please feel free to stop by and say hi!

CORNER FOR NURSE INFORMATION
Dear Parents:
I would like to welcome you all and your students back to school. I am looking forward to working with your children this school year. There are a few things I would like to cover:
Health Concerns: Please let us know if you child has severe allergies, health conditions that require monitoring is school (asthma, diabetes), or any health related restrictions/concerns. Also, please provide the school with the appropriate care plan/physicians orders.
Medications: Students will not be administered any medication without a doctors order and parent permission. This includes over the counter medications like Tylenol and Ibuprofen. If your child requires medication in school because of a medical condition (inhaler, insulin, epi-pen) I will need a signed doctor’s order and a parent’s signature. A prescription label is not acceptable as it is not a doctor’s order.
If a child has a doctor’s order and a parent’s signature the medication may be administered in school in the health office. The medication needs to be brought to the health office by a parents in the original container. The school does not stock any medications.
Immunizations: Please get your child up to date with immunizations. 6th graders are required to have a Tdap vaccine and kinder students are required to have their final doses of DTaP, IPV, MMR and Varicella. Notices of requirement and exclusion will be sent home within the month.
Illness/Absences: Please contact the office staff if your child will not be in school. If your child has a fever he/she should stay home until they are free of a fever for 24 hours without medication. Until this time they are still contagious and can spread the illness to other students.
If your child is diagnosed with any of the following illnesses/diseases please notify the office and keep them home for the following lengths of time:
Strep Throat- 24 hours after the first dose of antibiotics
COVID-19- 5 days from the onset of symptoms or positive COVID-19 test
Impetigo- 24 hours after the first dose of antibiotics
Chicken Pox- 7 days or until lesions are crusted over
Scabies or Ringworm- 24 hours after the first dose of prescription medication
Pink Eye- 24 hours after the first dose of prescription medication
Head Lice- after the hair has been treated and nits removed
It is our job to keep students in school but if they are not healthy and feeling well, they will not learn. If your child is vomiting, has diarrhea, has a fever of greater than 100.0, or has any other contagious illness please keep them home. If they come to the health office with any of these they will be sent home.
Screenings: Per Colorado State Law all students in kinder, 1st, 2nd, 3rd, 5th, 7th, & 9th grades must have a vision and hearing screening. We will be conducting vision screenings September 8th & 9th. Hearing and height and weight will be done September 22nd for the MS and HS and October 20th for elementary students. If the tests are abnormal your child will be rescreened. If the test remains abnormal after the 2nd screening a referral will be sent home and you will need to follow up with a doctor.
Dental screens and fluoride varnish will be done for students’ k-5th grade. These will be held sometime in September as well as again in the spring as it is recommended that children get two fluoride treatments a year. This is a FREE service. More information will be sent out as the time gets closer.

Center School Districts obesity rate in students has been historically higher than the state average!
Help your child – and your whole family – eat healthy and stay physically active. The healthy habits your child learns now can last a lifetime.
What can I do to help my child stay at a healthy weight?
Help your child stay at a healthy weight by balancing what your child eats with physical activity. Two of the best ways to help your child stay at a healthy weight are to:
· Help your child and family eat healthier foods
· Be more physically active as a family
You are a role model.
Parents are often the most important role models for children. When you choose to eat right and be physically active, your child will be more likely to make those choices, too.
Plus, being active and preparing healthy meals together are great ways to spend quality time with your family.

Community Eligibility Provision
Dear Parent/Guardian:
Center Consolidated Schools is participating in a Universal Lunch and School Breakfast Program for the current school year 2020-2021. If your children attend (Center Consolidated Schools), breakfast and lunch will be available to them at no charge. All students enrolled at this school may participate in the breakfast and lunch program at no charge to them.
Studies have shown that children who are not hungry perform better in school. By providing lunch to all children at no charge, we are hoping to create a better learning environment for our students.
The school breakfasts and lunches that we serve follow U.S. Department of Agriculture guidelines for healthy school meals. The School Breakfast and Lunch Programs cannot succeed without your support; please encourage your children to participate in the school meal programs.
Meals will be served to all students at no charge regardless of the eligibility status.
