Help Needed! Discovery Day Donations – UPDATED LIST!
Can’t be at Discovery Day but would still like to help in a REALLY BIG WAY???? Please consider donating some of the items listed below. There is a collection box for donations in the school office. Please have all donations in by Wednesday, May 24th to allow enough time for the leaders to purchase their remaining items. Thank you!
Items Needed for Discovery Day
- 3 Pin-the-tail-on-donkey games
- 3 dozen cake size plates
- 1 box of rock (ice cream salt) salt
- 1 large box of regular salt
- 3 large bottles of vanilla
- Sprinkles – (all colors; must be new/unopened)
- Candy glitter
- Wilton cookie icing or small tubes of frosting (all colors)
- Ice cream toppings
- Sugar flowers
- Sugar pearls
- Cake decorations (any)
- Small plastic cake toppers
- 16 Round Foil Aluminum Cake Pans – deep ones
- Paper towels
- Plastic bags (QUART size MUST BE ZIPLOCK BRAND)
- Fabric markers
- Glue on gems
- Glass marbles/glass stone pieces from Dollar Store
- Fleece fabric – (12 in x 12 in squares or half yards) kid friendly styles, patterns & designs - solid colors are welcome too
- Small - medium sized home decor pillows (to use as pillow forms will not be returned)
- Pillow stuffing
End-of-Year Health Office Happenings
Purging Student Files
Portions of the temporary health records for students moving on to (middle school or high school) will be updated and/or deleted prior to the end of the school year. If you would like to come in to inspect or copy any records, please call the school health office. All immunization records, accident reports, and records of release of information contained in the permanent record will be kept as part of the permanent school records.
Heath Office Requirements for School Attendance
Below is a list of school health requirements for kindergarten, second, sixth and ninth grade students for the upcoming school year. The required forms can be found on the district or school website under Health Services.
Physical Examination Requirements
The Illinois school code requires that students entering school for the first time (early childhood, kindergarten, or first grades), students entering kindergarten, sixth and ninth grades, and transfer students must submit evidence of a physical examination and a complete record of immunizations.
The completed physical examination and record of immunization form is expected at the school health office upon enrollment/ registration and required by the first day of student attendance.
Appointment dates for obtaining a physical exam do not meet the school requirement.
Students who do not meet these requirements will be excluded and will not be permitted to attend school per district policy.
Immunization Requirements
Immunization records must show proof of immunization against diphtheria, tetanus, whooping cough, measles, mumps, rubella, polio and chicken pox.
Early childhood and sixth grade through high school students are also required to have the hepatitis B vaccine..
Sixth grade thru twelfth grade are required to have the Tdap vaccine.
Sixth grade and twelfth grade are required to have the meningococcal vaccine.
Dental Examination Requirements
Students in kindergarten, second and sixth grades must submit evidence of a dental examination by May 15th of that school year. Dental exams must be completed within 18 months prior to May 15th.
Vision Examination Requirements
Students entering Illinois schools for the 1st time (kindergarten or transfer) must submit evidence of a vision exam.
All required health forms are available at the school office or www.cusd200.org. The school nurse may be able to provide resources for obtaining health requirements.
Medication Pick-up
Any student with medication kept at school must be picked up by you or a designated adult by May 31, 2017. After this date and time all remaining medications will be discarded.
Each school year we must have a new doctor’s order to continue giving your child medication in school. All medical forms are available on-line at www.cusd200.org/healthservices

Wheaton North Summer Recreation for Children
Enrollment has opened for summer recreation in both Activities and Athletics camps! Enrollment is open to all elementary – high school age students who live in the Wheaton North attendance area. Grade levels are defined as the grade the student will enter in the fall.
