September 2017
Dear Parent/Guardian,
The New Jersey Department of Education School Health Services Guidelines require that the district Board of Education notify parents/guardians of the importance of obtaining physical examinations during the student's childhood.
The Westwood Regional Board of Education has adopted the policy of requesting these physicals be completed during early childhood (grades two); pre-adolescence (grade five); and adolescence (grade eight and ten). This physical examination must state what, if any, modifications are required for full participation in the school program.
The student medical examination must be conducted at the medical home of the student and a full report must be sent to the school. A medical home is defined by N.J.A.C. 6A: 16-2.2 as meaning a health care provider and that provider's practice site chosen by the student's parent or guardian for the provision of health care.
The Health History, Immunizations Update, and Physical Examination forms can be found on the district website, www.wwrsd.org, under health portal.
Should you have any questions or concerns, please feel free to contact me. Have a wonderful weekend.
Sincerely,
Anna Parke, RN, BSN, CSN
