DISTRICT NEWS - PLEASE COMPLETE YOUR STUDENT'S CERTIFICATION FORM BY CLICKING ON THE LINK STATED IN THE CORRESPONDENCE BELOW
Dear Sandwich CUSD #430 Parents, Guardians, and Students:
With the first day of full in-person learning scheduled for August 19, the following update pertains to revisions to the District’s 2021-2022 Return to Learn Plan.
As you know, on August 4, Governor Pritzker announced that “effective immediately” all school districts are required to have masks worn indoors, thus all individuals must wear a face mask when inside a Sandwich CUSD #430 building or on a bus. This mandate is in place for all individuals, regardless of vaccination status and applies to any school activity held indoors. This requirement will remain in effect until further notice.
Per previous correspondence, full in-person learning will resume and the staff and administration look forward to greeting students on August 19 (1:10 pm dismissal).
If interested, the DeKalb County Health Department conducts daily vaccinations. More information can be found at the following web site: https://health.dekalbcounty.org .
Prior to the first day of school, please complete a one-time online self-certification form for your student(s). In lieu of daily self-certification paper forms, please find the one-time student self-certification form by clicking on the link below.

All items included in this correspondence are subject to change based upon changes in applicable factors including an increase in COVID-19 rates or revised public health guidance/mandates.
Thank you again for your many efforts and the District is looking forward to greeting our student body in-person on August 19.
Sincerely,

NOTES FROM OUR NURSE
New Student and 6th Grade Health Documentation Requirements
Our upcoming orientation night provides a great opportunity for parents/guardians to gather all of your students’ required health documentation and provide it to the school nurse or administration staff prior to the deadlines, in order to prevent any possible exclusions from school or any school-sanctioned events later this year. Below you will find all information on requirements for new and 6th grade students with links to the forms, if needed. Most providers should have the forms in their office and provide them to you, however. As always, if your student has a special health concern, medication to be taken at school, or you have any further questions you can reach out to me via email aquiles@sandwich430.org. I look forward to meeting you all on August 18th!Health plans for certain medical conditions (ex, asthma, diabetes, seizures, food allergies, etc.) need to updated every school year. If you have any questions or concerns, please contact the school nurse. REMINDER :Students who require an inhaler at school should also have a spacer/chamber available to be used with the inhaler
Medication Reminder
Any student requiring medication during the school day needs to have a completed school medication form on file in the office. This includes all prescription, over the counter, inhalers, and emergency/rescue type medications. Forms can be found on the district website or you may contact the school nurse.
