Transition Meeting

Welcome to Serendipity Elementary!
We understand that you probably have many questions and concerns as your daughter Kim prepares to join us in kindergarten. Starting school is a special time in every child's life, and it is our greatest desire that we start off on the best foot possible with all of our new students. I hope that the information in this newsletter helps to address some of your concerns regarding the upcoming transition meeting. Observation
Mr. Tiverton will be observing Kim's strengths and areas of need and matching those with specific strategies and programs to help her transition smoothly into kindergarten. He has had over 10 years of experience with children with special needs and is familiar with a variety of approaches and resources that could be of assistance.

What is the process?
2. If you have any relevant documentation (i.e medical, speech and language, physical assessments) please bring them to the meeting as these reports may help us to create the perfect plan.
3. Goals will be discussed and adjusted based on whether or not they appear to be achievable.
4. Supports and strategies will be put in place, and resources planned to assist Kim in achieving the goals that we set.
5. Each member of the team will leave the meeting with specific responsibilities in ensuring the plan is carried out.
