Consent to Invite Agency button LIVE!
You are now able to create a document in EdPlan for your 11th grade IEP's to go with the salmon sheet for consent to invite an agency. For now, we will be using both - we need the electronic document in EdPlan, but we also need the salmon sheet for the paper file. Continue doing the salmon sheets!
And... for 11th grade IEP's:
- go to Documents tab
- select radial for letter for 'consent to invite agency'
- click draft
To create your document:
- under 'name of transition agency' type in MRS
- under 'description of agency services', you can put "agency who may provide information on career/employment, independent living, community participation and post-secondary education and training opportunities"
- check the box for permission
- return response date = IEP date
- create draft
Take that document to your IEP - have parent sign
finalize this document when you finalize your IEP
Attach this doc to a salmon sheet (you don't need to fill that in, but we need it attached for the visual in the paper file audit).

Medicaid monthly service reports are due to Kim today.

Did you finalize all your outstanding documents before you finalized the IEP? Check for REEDs, MET's, etc... that must be final before you create the final IEP.

Did parents double check the contact info on page 1? Address, phone, etc... Please feel free to forward hand written changes for this information to Kim, and she will add to the student file.
