2019-20 MTMS 7th and 8th Grade Athletes
Dear Parents/Guardians:
7th and 8th grade students who want to participate in athletics must have the following steps completed before the first tryout or practice. Students who do not have ALL steps completed will not be allowed to participate until all forms have been completed and fees have been paid.
Forms that must be turned into the office (available on the MTMS Website under the Athletics/Activity tab or you can click on the link below):
- KSHSAA Physical – dated after May 1, 2019 by a MD, DO,DCPA-C or APRN)
- KSHSAA Parent Consent form must be signed and dated by both the student and parent

Forms to be completed online through Skyward Access (Both parent and student will have to sign into their own Skyward accounts to complete):
- Acknowledgement of Risk
- Athlete Code of Conduct
- Concussion Form
Form to be complete online by parent only:
- Emergency Medical Authorization (EMA) Form
Fees to be paid:
- $7.00 Catastrophic Fee – all student athletes
- Activity Participation Fee
- 7th grade - $30.00
- 8th grade - $35.00
Deadline for fall athletes to complete the athletic registration process is August 6th (to allow time for processing). Fall Athletics includes Volleyball, Cross Country and Football.
Yearbook Distribution
If your student is absent on that day they may come by the office to pick up their yearbook.
Attention 6th Grade Parents
Vaccination requirements are changing for the 2019-2020 school year!
According to the Kansas Department of Health and Environment, the new requirement states:
Meningococcal (Serogroup A,C,W,Y): Two doses required. Doses should be given at entry to 7th grade (11-12 years) and 11th grade (16-18 years).
6th grade parents: Since this is a new vaccination requirement, ALL students currently in 6th grade will need to provide proof of one dose of meningococcal vaccine to start 7th grade in the fall of 2019. The second dose will need to be administered before your student starts 11th grade. This vaccine helps protect your student against bacterial meningitis.
Your shot record might say MCV, MCV4, MCV40, MCV4P, Meneveo or Menactra. All of these will meet the new requirement.
In addition, ALL incoming 7th grade students need to have a Tdap (tetanus, dipertheria, and pertussis) booster shot.
Vaccinations can be obtained at your child’s doctor’s office, most pharmacies (like CVS and Walgreens) as well as the Johnson County Health Department.
If your student has already had these vaccines or when your student receives these vaccinations, please send the records to your nurse ASAP. You can send records to school with student, mail, e-mail or fax records to the address listed below.

The Meningococcal vaccination is currently listed as non-required, due to the recent change Skyward has not been updated to list as required
The Tdap vaccination may be listed as Tdap booster
