If the reason for the absence is due to illness, please indicate the symptoms your child is having - this is to insure accurate reporting to the health department when needed.
If your child is going to be late due to an appointment and would like to order hot lunch, please indicate your child's lunch choice.
Families planning to take children out of school for planned vacations should fill out an Oconomowoc Elementary Schools Family Vacation Request Form - at least two week in advance whenever possible.
REMINDER FROM THE DISTRICT NURSE ABOUT MEDICATION
Dear Parents/Guardians-
We understand that prescription and non-prescription medication may need to be administered to students at school. To ensure that this is done safely and efficiently by our trained staff, we want you to be aware of the requirements listed below. This policy is in accordance with state requirements set forth in Wis. Stat. sec 118.29. and Oconomowoc Area School Board Policy 453.4
- All prescription medication requires a parent/guardian AND Physician (licensed Practitioner) signature on the Consent Form for Medication Administration, before staff can administer the medication to the student
- Prescription medications must be provided in the pharmacy-labeled package. The label must list the student name, name of the prescriber, medication name, dose, and directions.
- Non-prescription medication (over the counter) requires only a parent/guardian signature, as long as the dose is within the recommendations listed on the package. Doses that differ from the recommended therapeutic dose as listed on the package require written authorization by a physician before they can be administered.
- Non-prescription medications (over-the-counter) will need to be in the original manufacturer package. The package must list the ingredients and recommended therapeutic dose in a legible format.
- Changes in dosing, type, or timing of prescription medication must also be made in writing with parent/guardian and Physician (licensed Practitioner) signature on the Medication Stop/Change form.
- With the exception of some emergency medications and other specific circumstances, all medications are to be stored in and administered from the health room.
Staff will not be able to administer medication to students unless the above criteria are met. To prevent delays and challenges for your family and student, we recommend bringing copies of the necessary medication consent forms to your student’s doctor appointment so that the required paper work can be completed in advance of the medication being brought in to school. WHAT IS PBIS AND HOW DOES IT WORK AT SUMMIT?
At Summit, we have used a system called Positive Behavioral Interventions and Support (PBIS) for the past few years. It is designed to enhance the overall school culture in our classrooms, throughout the building, and on the playground. We often refer to these as our Top Dog behaviors. As a staff, we're working to create consistent expectations and responses to student behavior with an emphasis on verbally acknowledging positive behavior. We recognize our overall student population behaves very well every day, and this initiative is intended to make the Summit experience even better for every child.
We've created classroom and school lessons to explicitly teach how to be a Top Dog across different areas of our school. Staff helped to create a matrix describing these behaviors and you will see signs posted in various areas explaining how to be a Top Dog. As students demonstrate their understanding of these expectations, the staff is poised to recognize and acknowledge their successes. We look forward to many more school-wide celebrations this school year!
One of the things we are using is using a process called 1) Remind, 2) Reteach, 3) Think Sheet, and 4) Office Discipline Referral (ODR). If a “minor” behavior is repeated a staff member will Remind the child of the expected Top Dog behavior. If the behavior continues, a staff member will Reteach the expected Top Dog behavior to the child. If the behavior continues, the child will fill out a Think Sheet (with teacher support when necessary) and the sheet will be brought home for a parent signature. If the behavior still continues, the child will visit the principal and an ODR will be completed and communicated with parents. If a "major" behavior occurs, a staff member will fill out an ODR, the student will process about their behavior with Mr. Stuckey and the information will be shared with parents. The Think Sheets and Office Discipline Referrals serve as a communication piece for families and a point of data collection for our staff. The goal is for all students to learn from the choices they make.
We know our students enjoy the lessons and acknowledgements emanating from the PBIS initiative. This initiative is part of a district-wide effort to improve academics and behavior throughout Oconomowoc, and Summit has been recognized at the state level for our successful implementation over the past few years.
