6th Grade Health Screenings - January 21, 2020
If you do not desire these services for your child, you must write a letter to the school. In your letter, state which of these health screenings you do not want your child to receive. Be sure to indicate your child's name and grade. The letter must be signed and dated by the child's parent or legal guardian.
Attention 6th Grade Parents - Required Shots BEFORE Entering 7th Grade
The State of Florida requires proof of immunization for all students entering 7th grade each year. This proof must be on the official State of Florida Immunization Form (DH680... the Blue Card) which is obtained from your doctor's office or the Health Department. The following immunizations must be documented on this form:
* Tetanus, Diphtheria, and acellular pertussis (Tdap) - This is the update shot that you need for 7th Grade.
* Hepatitis B vaccine series
* Varicella vaccine or the year of disease
* Two doses of MMR (measles, mumps, and rubella)
Please see your doctor or the Health Department for more information and to assure that your child has met these requirements. This documentation must be submitted to the school before the first day of school next year (August 10). Students will not be allowed to come to school until these shot requirements are met. Thank you for your prompt attention to this matter.

Attention 8th Grade Students
The 8th grade team is planning a trip to Gradventure 2020 at Universal Orlando on Friday, May 1, 2020. Gradventure is an opportunity for 8th graders to celebrate with their friends as they enjoy complete and private access to Universal Studios and Universal’s Islands of Adventure. There will be live DJs and street entertainment along with the attractions and rides.
We are scheduled to depart MSA on Friday May 1 between 1:30 and 3:00 p.m. and will return between 2:00 and 3:00 a.m. on Saturday, May 2. The cost is $140, which includes admission to Gradventure, a meal voucher to be used before 11:00 p.m. at select quick service locations, an 8th grade team shirt to be worn that night, and chartered bus transportation to and from Universal. Annual passes may not be used. Spending money for additional food, drinks, and souvenirs is optional.
Students must be in attendance on Friday, May 1, and all class fees must be paid in order to attend this field trip. Students should not bring backpacks to school that day as they are not permitted on the bus or at Universal. Drawstring bags, fanny packs and purses no larger than 8.5” x 11” are allowed.
Students must wear their 8th grade team shirt and must follow Universal’s Dress Code for Acceptable Attire attached to this packet. We will conduct our own security and dress code screening at school prior to departing for Orlando.
Please complete and sign the attached permission slip/medical form and return it with $140.00 cash or check (made out to MSA) no later than Thursday, March 5th 2020 to Mrs. Brebaugh in Room 904, or Mrs. Wallin in Room 803.
Additional information can be found at UniversalOrlandoYouth.com/Gradventure
New - MSA Outerwear Letterman Jackets
As you know, for increased security on campus, MSA does not allow any non-uniform clothing in secured areas. For students who are interested, we now have available MSA Letterman Jackets that can be ordered and customized with your child's name on them.
There are middle school and high school jackets, so make sure to click on the correct one as you order. Sample jackets are displayed in the school store, but all jackets must be ordered through our uniform partner, Manatee Apparel Graphics.
