Student Fees and Forms
Also found on your child's PowerSchool account, under the "Forms and Fees" tab, are many of the consent forms that have been sent out on paper in the past. These forms include the annual field trip consent form which needs to be completed before your child will be allowed to participate in any off-site school activities. Please complete these forms as soon as possible as we have many classes who have field trips planned for next week! Please see our school website for further instructions.

If you are experiencing financial difficulties, EICS is offering a Fee Waiver Program again this year.
Please visit: https://www.eics.ab.ca/download/77800 to download a copy of the forms to fill out.
Please not that all applications must be received at EICS by December 12th, 2016.
Emergency / Medical Forms
Medication will only be accepted with fully completed forms. EpiPens – Capital Health recommends that children who have severe allergies and require an EpiPen carry these EpiPens with them at all times. Many children carry an EpiPen with them in a “fanny pack”. We would like to see all children carry their EpiPens with them; however, some parents prefer that the EpiPens be stored in the office area. If your child will not carry his/her EpiPen, we ask that you provide two EpiPens: one for the office and one for the classroom. If you are only supplying one EpiPen, please submit a letter indicating this. Students may carry their inhalers in their backpacks or store in the office.
Grade Three SLA's "Student Learning Achievement"
Grade 3 students will be participating in Alberta Education Student Learning Assessments in October. This assessment enables teachers to learn about and identify student strengths and areas of growth at the beginning of the school year. Information will be sent home with grade 3 students very soon. If you have questions, please contact your child's grade three teacher: Ms. Stanna Kelava, Mrs. Lisa Morin, Mrs. Yvette Morin, or Mrs. Kerri DeChamplain.
