5 Essential Survey
Addison School District 4 participates in the annual Illinois 5Essentials Survey that collects feedback from students, teachers, and parents each school year. Your participation in the parent survey will enable you to share your thoughts on important elements of our school's effectiveness. All responses are confidential and will never be connected to you or your child.
To take the survey, please visit https://survey.5-essentials.org/Illinois/ and select the appropriate survey.
For parent results of this survey to be included in our ISBE designation for this school year we need 20% of parents to take the survey Please strongly consider taking this survey as we want your feedback and want to exceed the 20% response rate! We currently have 12%!!

Kindergarten Registration for the 2021-22 School Year
Beginning on March 8, 2021, parents may go into the www.asd4.org/registration/ website to enroll or register their incoming Kindergarten Student. All forms will be completed online!
In addition to the required online registration, parents must provide the following:
- 4 proofs of residency (one must be a mortgage, rental agreement or attestation)
- Original birth certificate
- Immunization/physical form (must be turned in before the first day of school)
Schools will be open to complete registration by turning in proofs of residency, birth certificate, etc., on the following two dates for incoming Kindergarten families:
- March 24, 2021 from 9am to 7pm
- April 10, 2021 from 9am to 2pm
Appointments for these two dates will be made after the online registration is complete or by calling the school.
If a language other than English is spoken in the home, the child must have an assessment of their English Language Proficiency to determine correct placement in either General Education or Dual Language. The district is sponsoring an assessment date at Indian Trail Junior High on April 17 from 9-2. When the online registration is complete and additional registration information is submitted, an appointment will be made to have the child screened for English Proficiency on that date.
The P-EBT and You
Serving the families in our community!
The State of Illinois is approved to provide new Pandemic Electronic Benefits Transfer Program (P-EBT) benefits to all eligible Illinois students. Eligible families do not need to complete a P-EBT application. However, families are encouraged to complete a Household Eligibility Application to determine eligibility for free or reduced meal status if they have not already for School Year 2020-21. It is also essential to ensure your mailing address on file at your enrolling school is current. Please note that citizenship status does not affect eligibility.

Virtual Science Night- DATE CHANGE
The safety and well-being of our students and staff are most important to us. Teachers, staff, and administrators will be receiving their second Covid-19 vaccine on March 11. Due to the unknown side effects, they may encounter, and our desire to have the most fun possible, we will be pushing back Science Night to March 18. If you are already registered and able to participate on On the 18th, there is nothing you need to do! More information to come as the night gets closer. Thank you for your understanding.

All Hybrid Students Self Certify by 8:00 AM
We are now asking that all Hybrid students self-certify by 8:00 AM every day even if they attend in the afternoon. This will help our office staff and teachers to be prepared for students. Thank you for understanding.
International Travel Reminder
As we prepare for hybrid students to return back to school beginning on Tuesday, January 19, we would like to remind families regarding our safety protocol for international travel. For students who have traveled outside the country over winter break, the student must quarantine for 14 days beginning the day after the student arrived back to the United States BEFORE they can attend school in person. This precaution is done to ensure the safety of all of our students and follows the CDC guidance regarding COVID-19. If you have any questions, please contact your building principal.

Student Attendance is MANDATORY!
Remote Learners will not be considered "present" if they log on but are not on camera and are not participating. Students that are not showing up, logging in, participating and engaging will be marked absent and, in some cases, truant. In extreme cases of absences or truancy, a home visit will occur by Ardmore staff which could include the Addison Police Department. In extreme cases, students will be reported to the DuPage County Truancy Program.
ATTENDANCE IS MANDATORY, AND PARENTS HAVE A RESPONSIBILITY TO ENSURE THEIR CHILD ATTEND SCHOOL REGULARLY!
