Hello!
My name is Regina Miller and I am very excited to be your new nurse at Liberty Elementary School. Born in Dallas, I am a native Texan, a Flower Mound resident, mother, and grandmother. I also have "two" many dogs and am happy to be here after spending the last 19 years as a Registered Nurse in the Dallas Independent School District. I am a graduate of The University of Texas Health Science Center at Houston School of Nursing and have been a Nationally Certified School Nurse since March 2000.
Healthy kids learn better, and I believe a good relationship with parents is crucial, so please feel free to email, call or drop by the clinic and introduce yourself if there is anything you'd like to share about your child.

Immunization Information
Students may not enroll, or continue attendance at school, unless the minimum state vaccine requirements have been met. Exclusions from compliance are available on an individual basis including medical necessity or reasons of conscience.

Medication - What Do I Need To Do?
A lot of kids need medicine while they're at school, but in most cases elementary school students are not allowed to carry medications with them. Medications are kept in the clinic and administered by me or a staff member designated by the Principal.
The child's condition must necessitate that the medication be given during school hours.
We can not administer any herbal medication or oils. Also, for the safety of all students, elementary children may not transport medications to and from school or keep medicine or cough drops in their backpacks.
All medications must be in the original, current, properly labeled container with clear and legible instructions.
The following Parent Request for Administration of Medication by School Personnel is needed for all medications and must be signed by your doctor or healthcare provider. In the case of multiple medications, one form is required for each medicine.
A physician's signature is required on the Parent Request for Administration of Medication by School Personnel form for:
Over-the-counter medications given more than 5 school days.
- Prescription label does not match the parent request or is missing required information.
- Medication samples or off-label prescription requests.
Medication can bring lots of questions and I am always available to answer any questions you might have.
