Health Office : Re: 8th Grade Vision Screenings
From the Health Office:
Dear Parent/Guardian of 8th grade student/s:
All eighth grade students will undergo vision screenings mandated by the State of Illinois during the months of December 2017 and January 2018. Following the screening process, parents/guardians of students requiring a comprehensive professional vision examination will be notified.
Please keep in mind the school vision screenings are not designed to be diagnostic and are not a substitute for a professional eye examination. Your child is not required to undergo the vision screening if a professional eye examination has been completed in the previous 12 months. To be exempt, a report completed by your child’s eye physician must be provided to the school nurse prior to December 1, 2017.
If you have any questions or if you need any further information, please feel free to contact the Health Office.
