Middle School Clinic Updates

Required vaccines for 7th-grade entry include a Tdap AND MCV4 (meningitis). A completed shot record is due to Mrs. Turpin, no later than Monday, Sept. 19th.


Those students who do not have a completed shot record on file will not be able to attend school after Monday, September 19th if this documentation is not received.


Completed records can be emailed to mturpin@northmontschools.net or your student can bring them to the clinic.

Walk-In Vaccinations

The Public Health – Dayton & Montgomery County

Immunization Clinic, located at 117 S. Main St. in Dayton, will be offering walk-in hours for back-to-school immunizations on the following upcoming dates:

Wednesday, September 14; 12:30 – 4:00 pm

Wednesday, September 21; 12:30 – 4:00 pm

Wednesday, September 28; 12:30 – 4:00 pm

Walk-ins will be taken until the clinic capacity is reached.

This is to ensure each of our clients receives adequate

attention for their appointment time. You may also call

the Clinic to schedule an appointment at (937)225-4550.


NOTE: A parent or legal guardian must accompany

children to their appointment. You MUST bring the

following items with you to your appointment:

• Immunization (shot) record

• Insurance card(s) - Many insurance providers, including all Medicaid plans, are accepted for immunizations. No child is turned away if the cost is unaffordable. • If the person bringing in the child is not the legal guardian/parent, download the Designation of Another Person to Consent for Treatment Form at: https://www.phdmc.org/client-services/immunization-clinic

• Any custody or guardianship papers (if applicable)

• Photo ID for parent/legal guardian

• Wearing a face mask is required

Friday Night Football Expectations for Middle School Students

1. If students are attending the game without a guardian, they can be dropped off at Door 1 at the Middle School to avoid stadium traffic. Students can then walk over to the stadium.

2. Students are expected to be seated in the stands, being respectful, responsible, resourceful, and kind spectators.

3. Students must remain in the stands until the last 2 minutes of each quarter. At that time students may go to the restroom and to the concession stand, and then return to their seats.

4. An announcement will be made at 9:15 p.m. for all middle school students to leave the stadium and report to Door 1 at the middle school.

5. Pick-up time for all middle school students is 9:30 p.m. (Door 1 - Middle School)

Students must leave at this time even if the game is not over unless their ride is at the game. If the student's ride is at the game, the student will be allowed to remain in the stadium.

6. Students who are not following our expectations will be asked to leave the game and a phone call will be made to a guardian to pick the student up, refunds will not be issued.

7. We want the game to be enjoyable for all spectators.

Is your middle schooler interested in video games? What about trading card games? How about chess, anime, and board games? Well, Nerd Herd is the place for them. Nerd Herd meets on Mondays afternoons from 3 - 4 pm. Families must provide transportation home.

Volunteer Information

Northmont Middle School has an 8th-grade class named Career Education. This class helps students identify potential career pathways they may pursue including college, trade school, and the military. The Capstone project for this class is a Job Shadowing experience. Job Shadowing involves spending time following a professional performing their day-to-day duties in a work setting. Northmont Middle School is looking for adults who are willing to participate in our Job Shadowing efforts. Please read through the Employer FAQ document as well as the Interview Questions that will be asked. If you are interested in volunteering to be a Job Shadowing host, please fill out this Form. Your participation is greatly appreciated.

Free/Reduced Meal Applications can be completed and submitted in 2 different ways:

  1. Online. You must register a student account with PaySchools Central in order to fill out the application online. Once the account is set up and you are logged in, locate the menu dropdown icon on the dashboard and select the Free/Reduced Meal Application option. Then follow the instructions on the screen.

  2. Printed. Applications can be printed from the Northmont Food Service website, picked up at the student’s school building or Food Service Office, completed in its entirety, and returned to the school building or Food Service office.


School Meal Pricing 2022-2023: Breakfast: $1.90

MS/HS Lunch: $3.00
