Transition Assessments: Parent Consent
- If you are gathering a significant amount of new information because you are looking at a new area of concern or determining what services to provide that are new - this is likely a reevaluation situation and signed consent would be required. For example, if you want to have the student complete 3 trial job placements for 1 week each to gather data to determine what skills/experiences the student needs next. This may require a significant amount of assessment and potential changes to services.
- If you are gathering new assessment information that isn't part of the activities of the classroom/program then you would want to document parent permission on the IEP or prior written notice. For example, the teacher wants to administer a specific test to gather more updated information in an area they have already supported.
- If you are getting to know your students as part of being in the secondary special education program including gathering information about their skills, interests, preferences for living, learning, and working then no permission is needed. For example, things like simple checklists or interest surveys could be part of the program and not require additional consent.
Exiting Special Education Services (Graduation)
When a student exits special education due to graduation from secondary school or due to exceeding the age eligibility for FAPE, the student must be provided a summary of his/her academic achievement and functional performance which shall include recommendations on how to assist the student in meeting his/her post-secondary goals. [41.305(5)c]
Either a Summary for Postsecondary Living, Learning, and Working (SOP) form (SOP Template), or a Support for Accommodation Request (SAR) form (SAR Template) must be completed to meet this requirement. Please take the time to review the videos below to assist you in the completion of either form.
