Sharing Food At School is Not Permitted
We are demonstrating we are safe when we eat our own food or we say 'No thank you" when we are offered food by another person. We appreciate you reminding your child that sharing food at school is not permitted.
The District 15 Board of Education unanimously approved a resolution to place a bond referendum on the November 8, 2022 ballot. Doing so will allow the District to repair and restore our facilities, bring full-day kindergarten to all elementary schools, update schools libraries and some learning spaces, and ensure feeder patterns from elementary to junior high and high school providing all student access to a neighborhood or near neighborhood school. To learn even more about the proposed improvements visit www.ccsd15.net/Moving15Forward.
District 15 will be hosting informational meetings on the following dates:
October 6 - Thomas Jefferson Elementary School (Thomas Jefferson Middle School if the measure is approved)
October 11 - Winston Campus Junior High
October 13 - Plum Grove Junior High
October 20 - Carl Sandburg Junior High
October 25 - Walter R. Sundling Junior High

Health Forms
For the purpose of safeguarding the health of children, District 15 enforces Illinois student health requirements. These requirements consist of health, dental, and eye exams and immunizations specific to grade and age.
Health exams are required for students entering pre-school, kindergarten, sixth grade, and those who are new to the school. The Certificate of Child Health Examination form is available online and in the school office.
The exam form, completed and signed by a licensed healthcare provider, must be dated within one year prior to the first day of school and must include lead screening for children who are six years or younger and diabetes screening.
Health forms can be brought to the school office at any time.
On October 15, students who have not provided a health exam or required immunizations will be excluded from school. Students new to Illinois schools, who register mid-term, have 30 days following registration to comply with the health requirements.
For more information on specific health forms needed, visit the Health Services website.
