Peak View Behavioral Health invites you to attend a presentation by Clark Flatt of The Jason Foundation, Inc. for the prevention of the "Silent Epidemic" of youth suicide.
The Jason Foundation, Inc. is dedicated to the prevention of the “Silent Epidemic” of youth suicide through educational and awareness programs that equip young people, educators/youth workers and parents with the tools and resources to help identify and assist at-risk youth.
Peak View Behavioral Health is an affiliate office of the Jason Foundation.

Clark Flatt is a nationally recognized speaker on the topic of youth suicide awareness and prevention. With over 20 years in the field of suicide prevention, Mr. Flatt, as President of The Jason Foundation, Inc., leads one of the nation's oldest and largest non-profits whose mission is addressing the national health crisis of youth suicide.

The Affidavit of Residency is a mandatory form that needs to be completed and turned in ASAP.
