The Purpose of a Transition Meeting
We want to help Kim prepare for Kindergarten by having a transition meeting. We will discuss your child's strengths and needs to better prepare for her arrival to school. We can plan next steps and goals for your child with your input. We can also identify any extra supports your child may need as well as introduce some of the school staff that will be working with Kim.

Ms. Johnson, our principal will call you to make sure you have the needed documents to review before the meeting. Please call the Principal if you have any questions prior to the scheduled meeting.
When you arrive for the transition meeting, Ms. Johnson will meet you at the office. Please let us know if you are unable to make the above scheduled time. We will be happy to reschedule at a time that is convenient for you.

Who Can Attend the Transition Meeting?
Parents/Guardians, Classroom Teacher, Principal, Special Education Teacher, Educational Assistant, Occupational Therapist, Physiotherapist and Speech and Language Therapist
What will The Process Be?
1. Preparation:
- Organizing the Transition Team Members
- Orienting the Transition Team and reviewing all documentation
- Input from all Transition Team Members
- Identify Student's Transition Goals
- Identify the steps and actions needed to reach the goals
- Coordinate the Transition Plan with the IEP and Health Plans
- Identify Timelines
- Recording the Transition Plan
- Filing and Distributing Copies of the Plan ( A copy of the IEP and the Transition Plan will be given to the parents/guardians
Roles and Responsibilities
Student: A personal commitment to the Transition Plan is important for its success. This is enhanced by their meaningful involvement.
Parents: Provide support for their child and help communicate information on their child's needs, strengths and needed supports within the classroom.
Principal: Ensures that the transition plan is developed and maintained.
Teachers: Instruct and assess the student using instructional strategies and accommodations/modifications listed in the IEP
Special Education Teachers: Identify appropriate goals for the student and the tools and strategies to help them reach those goals
Educational Assistants: Support the student in their learning, social skills and personal care needs
Special Education Administrator: Support students with complex needs and arrange needed services and supports
Community Services and Health Care Providers: Support students in school and at home such as the CCAC, Community Care Access Centres. Referrals can be by the school for health care and social support.

- Give the student the support needed for cognitive, social, emotional and physical development
- Facilitate strong communication between home and school and social services
- Build on a student's strengths to reach his/her potential
- Provide educational needs as well as healthcare needs
