FROM THE SCHOOL NURSE
Welcome to Heritage Elementary!
My name is Marni Dreher and I am the School Nurse at Heritage Elementary and Pickerington North H.S. I look forward to meeting your child and working with you. I wanted to make you aware of some kindergarten health requirements.
1. Immunization Record - The State of Ohio requires all Public-School children to have updated immunization records. Please see Ohio Immunization requirements:

2. Physician Report form - A current completed form, signed by a HealthCare Provider, dated from 08/19/21 to present, is due by August 1, 2022. The Physician Report form can be found here:
If you have not turned in your child's immunization record and physician report form upon enrollment, please email the forms to school nurse's email marnica_dreher@plsd.us
If you have already turned the forms in upon enrollment, the information has been received and you do not need to send it again.
Health Concerns - please communicate your child's health concerns and medications, if needed, a health plan will be developed and shared with teacher(s).
Change of Clothes - During school hours your child may need to change their clothes due to a bladder/bowel accident and/or falling in water puddles and/or mud. If possible, place an extra set of clothes in their book bag to use.

COVID Protocol
The following are the guidelines PLSD is following from Franklin County Public Health Department regarding COVID guidelines.
COVID Test positive -
The student should stay home for 5 days from the onset of symptoms.
The day the student started having symptoms is day 0.
The student should return to school on day 6 as long as the positive person is no longer running a fever, not taking any fever reducing medications and has improvement in symptoms.
It is recommended that the student wears a mask when indoors and around others for days 6 - 10 and unmask on day 11.
For student attendance, to be marked as medically excused, the parent will need to provide documentation of:
- a proctored COVID test result or
- a note from the healthcare provide
The recommendation for family members and anyone living in the household where there is a positive case:
- if no COVID symptoms, the individual may return to school wearing a mask for 10 days from last contact with the positive person.
- if possible, test on day 5
- if you develop symptoms, get tested and stay home until you know the results.
