How to Pick Up Medicine Left In the Clinic
If your child still has over the counter and/or prescription medications in the clinic, you will be able to pick these up on the day you come to collect their school belongings. Nurse Shepherd will be in the loops during the designated pick up times on May 18th and 19th. Parents should make every effort to pick up medications during the times available.
Please fill out the Google form below to schedule your medication pick-up.
Medications will not be released to a person other than the parent or legal guardian, unless the parent designates the pick-up person on the Google form. Picture ID required for pick up. Any medications left in the clinic after the pick-up times will be destroyed.
E-mail Nurse Shepherd at cshepherd@coppellisd.com if you have any questions or concerns!
