May Special Message from the School Nurse
Vision and hearing screening results were sent home for students who were screened. If the student did not pass either screening, the parent/guardian needs to make an appointment with the child’s medical provider. The medical provider will complete the bottom of the form and the form must be returned to the school nurse. If you need another copy of the vision/hearing screening results, please contact Nurse Debbie at dchetelat@bcps.org.
We are required to have a physical on file for all students. We only need updated copies if there is a change in health status or treatment for a medical condition. We need one dental form filled out for each student as well. If there are any new vaccinations given, we should receive a copy detailing what they were.
For the spring, we highly recommend students keep a change of clothes in their locker. We have a few clothing items here, but we cannot supply all sizes for all the students when clothes are wet or muddy from outdoor recess or spills. Please feel free to send in any unwanted or unused clothing items or shoes. Whatever we can't use we will donate.
