Returning Students Annual Enrollment
Dear Parents/Guardians,
EPCUSD 401 2023-2024 student enrollment information is now available. Enrollment is a three step process that is done through PowerSchool.
Step 1. Prove you are a resident of Elmwood Park (residency)
Student residency is reviewed in the spring. If your residency was verified in public record, you do not need to submit residency. If it was not verified, you will need to provide proof of residency through the PowerSchool portal. Your PowerSchool account will be updated of your status. Students without approved residency will not be able to enroll in D401 schools. (login below)
Step 2. Complete enrollment information
ALL families need to complete all enrollment information in PowerSchool. This is how we maintain our updated student records. This includes the necessary medical forms needed.
Step 3. Pay school fees (notification will be sent when your invoice is ready to be paid)
School fees are paid online through MySchoolBucks. Additional information about the school payment system is here.
Enrollment Period:
First Enrollment Window: April 3rd-June 15th **Enroll now to secure your child's school placement
Second Enrollment Window: July 17th-Start of School

Medical Forms
The Illinois State Board of Education requires that families submit medical information prior to being enrolled in school. All medical documentation must be completed prior to starting on the first day of school. If your child is in Kindergarten, 2nd grade, 6th grade, 9th grade, or 12th grade there is required medical documentation for your child to enter school on the first day.
Loyola Pediatric Mobile Health Unit for families with no health insurance
The Loyola Pediatric Mobile Health Unit can see and do the following for any student that does not have health insurance:
- Children from birth to 21 years old (*can only vaccinate age 18 and under)
- Patients without insurance or state-funded insurance (Medicaid, etc.)
- School physicals (required before pre-kindergarten, kindergarten, sixth and ninth grades, and for new students to the school)
- Sports physicals (expire after a year)
- Immunizations
- Lab testing (hemoglobin, lead, random glucose)
- Hearing and vision screening (scheduled as a separate visit)
- Health education
Parent/Guardian will need to do the following:
- Complete the Loyola Health Care/Immunization Consent Form
- Complete the Loyola Mobile Medical Unit Consent for Release of Health Information
- Complete the Sports Physical Form (if needed)
