Attached are important parent information, membership forms, and Indiana kids form. Please read them and let us know if you have any questions. The membership form and Indiana kids form are both required to participate. The Indiana Kids form is part of additional grant funding that positively influences your member(s); we do need one completed per child. The membership fee and a weekly registration cost per child are as follows.

Membership fees – (6/1/2021 - 3/31/2022): *Membership does not guarantee a weekly spot for your child

*Covers attendance for: Summer of Exploration and additional breaks from school (ex. spring break, fall break, snow days).

1st child $20.00

2nd child $15.00

3rd child $10.00

Weekly fees – (per child): *Weekly pre-registration is required & is how to guarantee a spot for your child

Summer/school breaks: (6:00am - 6:30pm) - $125.00

-We offer a sliding fee scale for the weekly fees. Based on “Total” household income. If you would like to apply for this program do not forget to submit income verification from the approved list (page 3) on the membership form.

-If additional assistance is needed please let us know upon returning the forms so we can address those inquires specifically.

Includes - Breakfast (7:30-8:30am), Lunch (11:00-12:30), snack. If you are dropping your child off outside of these times, you need to provide a meal for them.

HOW: Download the REMIND APP enroll using class code | @FFYSOE (club & camp) | @FFYTEENS (teens)

WHY: Programming questions (activities, field trips, swim schedule, meal times, etc.)

Pick-up (changes to approved list, special drop off/pick up time, etc.)

Medication, specific needs for your child, checking in on your child during the day while on premises, etc.

By submitting a completed membership, you acknowledge:

- You have read the Parent Handbook

- You will adhere to all policy guidelines

- You will sign up for the REMIND APP so you can communicate directly with staff at CLUB/CAMP/TEEN

- You understand that membership does not guarantee a spot for your child.

- You will pre-register your child prior to them attending.
