Student Health Survey
· Student health and safety
· Student mental and behavioral health
· School climate and culture
Completing the Student Health Survey poses minimal risk to your child, who may feel embarrassed or uncomfortable answering some of the questions. Survey procedures have been designed to protect your child's privacy and allow for anonymous participation. If your child is not comfortable answering a question, he or she can leave it blank. While we encourage all students to participate, the decision to participate is voluntary. You may opt to send a refusal for your child’s participation. Alternatively, your child will have the opportunity to refuse at the time of the survey. There will be no action against you or your child if your child does not participate.
If you do not wish your child to take this survey, return this form to the principal’s office before the survey date of October 26-November 6. You need only return this form if you do not wish your child to participate.
The survey is available on the Oregon Health Authority (OHA) Public Health website for you to review before the survey.
If you have other specific questions about the survey, please feel free to call the main office, 503-356-2680.
Student Device Damage & Protection Plan Enrollment Deadline
The deadline to opt into the Device Damage & Protection Plan for families enrolled on September 14th was Wednesday, October 14, 2020.
If you opted into the program and have not paid the $20 fee please do so asap. Students need to pay the assigned fee prior to any damage or loss claims to be eligible for the Protection Program coverage and no later than 30 days after they begin school.
This $20 fee may be paid via the Online Payment System or by submitting a check and a completed form to the office. Checks must be made payable to Stoller Middle School (NO CASH).
This fee is waived for families who qualify for the free or reduced meal benefits program and have completed the “Permission to Share” form.
Attendance during Comprehensive Distance Learning (CDL)
To report all day absences or absences during the day due to appointments, contact Attendance for excused absences.
The Attendance line: (503)356-2681 or email: sms-attendance-shared@beaverton.k12.or.us
This email can also be accessed on Stoller's website.
If your student is going to be absent and not going to access curriculum anytime during that day, then you will need to report the absence with attendance for that to be an excused absence.
If your student is absent from a class and they access the curriculum, turn in an assignment, or engage in an activity on canvas anytime during the day, they Engaged Online (EOL) and this is not an absence, no need to call or email attendance. This is not considered an absence and the teacher will be able to make corrections in the students' attendance if they are marked absent. You may get an automated call from Stoller at 5pm, you may ignore it.
If your student was marked absent from a class that they did attend, please email the teacher to get it corrected. Do not call the attendance line, the teacher must confirm any changes to be made.
See the following link for an easy to follow flow chart for Attendance during CDL

AMC8
The AMC8 is a national contest and will be administered digitally this year on November 10th. Exact times will be announced in a future date. Families will be responsible for proctoring their student’s exam and providing reliable internet access. Registration fees are covered by the district. This assessment is designed for high-achieving and highly-competitive Mathematics students.
Any student who is interested should fill out this form. This form must be completed before October 21 to sign up for the official contest. Interested students will be asked to take a practice test. They will receive a confirmation email after submitting their form.
Please work with your student to establish realistic goals before they choose to take the official test. Students may instead choose to take the test unofficially and score their own work in a non-competitive setting. Practice materials for The AMC8 can be found here: https://artofproblemsolving.com/wiki/index.php/AMC_8_Problems_and_Solutions
Please contact Mr. Nelson directly should you have any questions and/or concerns.

Stoller Volunteers
Welcome Stoller Volunteers!
Volunteering in the 2020-21 school year will be unlike previous years. Although we are unsure what remote volunteer needs will be, we invite you to create a volunteer profile or update your existing profile. This will ensure you are able to help Stoller staff and students through volunteer work as opportunities arise. A volunteer profile is required for all volunteering within BSD schools. Please follow the steps below to update or create your volunteer profile. Additional information is available at www.stollerpto.com/volunteer.
Existing BSD Volunteers (new to Stoller)
Please send an email to update_volunteer_locations@beaverton.k12.or.us and request to add Stoller Middle School to the organizations listed in your mVP account. Provide your full first and last name as it appears in your profile. If you will no longer be volunteering at an elementary school, you may also request to remove that school from your profile.
New BSD Volunteers
If you are a new volunteer to the Beaverton School District, you must create a volunteer account. This process includes a background check.
- Parent/Guardian: Complete the Parent/Guardian Volunteer Application
- Community Volunteer: Complete the Community Volunteer Application (This applies to Grandparents, Nannies, Neighbors, etc)
- Youth Volunteers: Complete the Youth Volunteer Application
Within the application, please include all of the BSD schools where you are a volunteer. You will have one profile for all your BSD volunteering.
- This step also initiates a background check. It will come up for renewal every 2 years.
- Once your background check is complete, you will receive an email from Stoller PTO stating your background check approval status, and your volunteer profile at myimpactpage.com will be active.
Thank you for giving your time and talents to Stoller this school year!
If you have any issues with your device or logging in, you will need to contact the Student Help Desk.
