myDSD - Parent Agreement for iPad Checkout
Parents can log on to their myDSD account and sign the Parental Device Permission Form electronically.
Steps for Parents to do this:
- Log into the myDSD system
- Click on the Tools tab
- Click on Sign Documents
- Select Parental Device Permission Form.

Pick up schedule for Wednesday:
Last name letter
A – E ……….. 8:30 – 9:30
F - K …………9:30 – 10:30
L – Q …………10:30 – 11:30
R– Z …………11:30 – 12:30
These devices should be used, by students, to complete teacher assigned curriculum and communicate with their teachers. An appointment can be made for you on Wednesday, March 19, from 4:00-5:30 pm if you cannot pick up the device in the morning. Please call 801-402-3500 between the hours of 8:30-1:30 pm for an appointment if you need a time outside this above schedule. We are staggering pickup times to maintain safe social distance.
