Hong Kong is a special administrative region of China known as the Special Administrative region. The most essential element of Hong Kong's healthcare system is the"rative" or healthcare system, and this system is accomplished through the implementation of an electronic health declaration form. All healthcare professionals in Hong Kong are required to complete this form prior to being granted some other medical treatment. The electronic health statement form was introduced in 2021 as part of the healthcare reform policy by the authorities of Hong Kong. Currently, it is one of the most frequently completed forms in Hong Kong.

This is largely because a lot of work is involved in filling in the electronic health declaration form. To begin with, you have to fill in the information about your name, address, contact details and your livelihood. Then you will need to include the details of your physician including his registration number, his name, name and his permit number. When you've completed the electronic health declaration form, you have to submit the same to the Department of Health. Your physician would then forward the completed file to a hospital of jurisdiction for submission of your health record. Once your health record is obtained, a certified health practitioner will review it and decide whether you are fit to get any healthcare services from that hospital. The doctor may also give you a copy of your health record together with a letter of evaluation indicating the current state of your health.
When you opt for an assessment, your physician will fill in the digital health declaration form and deliver it to the EHR service provider. The service provider then uses the details about the digital health declaration form to ascertain whether you're qualified for health care services in the hospitals in that establishment. If it is found that you meet the eligibility criteria, the healthcare providers in that hospital would provide you with the services that are requested of you. The whole process usually takes less than 1 hour.
When you complete the electronic health statement, you need to sign it also. This is imperative to ensure that you understand all the terms and conditions stipulated in the record. In case of any queries, you can seek support from the service provider or your clinic. Typically, the information supplied in the digital health statement forms are true.
However, in case there are modifications to the eligibility criteria, you will have to upgrade your electronic health statement as soon as possible. Along with this, you should ensure that you get a copy of your digital health record until you leave the country. You should keep your electronic health record in a secure place and update it at least once a year to reflect any adjustments made to your eligibility criteria.
