Letter from our New Principal
June 1, 2018
Dear Forman Elementary Families,
I am honored to have been selected as the new principal of Forman Elementary. I feel truly blessed to be the instructional leader of such a great school, and wanted to share a little about myself.
I grew up in Houston, Texas, after emigrating to the United States from Colombia. As a graduate of the University of St. Thomas in French and International Studies, earning a Master’s Degree from the University of Houston in Bilingual Education and later from UT Dallas in Administration, I have dedicated over 25 years’ total to the field of education. My experience as an English teacher in Japan, in Houston and Richardson ISDs as a bilingual teacher and later in Dallas ISD and Richardson as a principal have really solidified my love, dedication and passion for being a bilingual instructional leader. I have also had experience as a district Bilingual and ESL Director, as well as a bilingual grant coordinator. Working with historically underserved populations is a challenge that brings me great personal and professional satisfaction.
I am very much looking forward to meeting the dedicated families and community of Forman Elementary. Along with the staff at Forman, I will continue the Falcons’ legacy of success by supporting teachers, parents, and students. When facing new challenges this school year, I will always base my decisions on what is best for children. I look forward to the opportunity to meet you and your children in the coming weeks.

Parent Notification of Student Performance
State of Texas Assessments of Academic Readiness (STAAR)
Plano Independent School District
Spring 2018 State of Texas Assessments of Academic Readiness (STAAR) results for students in grades 3-8 will be available for Plano ISD students beginning June 13.
Parents/guardians will need to go to the TEA Texas Assessment Data Portal http://texasassessment.com to access the Student Report Card which is the official score report provided by the state. Please see the attached flyer that includes the steps to accessing your child’s STAAR performance.
To view the Student Report Card you will need to enter your child’s unique Portal Access Code.
You may locate your child’s unique Portal Access Code directly on the TEA Assessment Data Portal or on the PISD Parent Portal.
OPTION 1 - Go to the Texas Assessment Data Portal http://texasassessment.com
You will then be prompted to enter in:
-Student’s First Name,
-Social Security Number or S-Number (State issued ID Number), and
-Date of Birth.
OPTION 2 - Go to the PISD Parent Portal https://parentportal.pisd.edu/Login.aspx
Login to the PISD Parent Portal.
Scroll down to Performance Management.
Locate the STAAR PORTAL CODE.
If you have any difficulty accessing the online information for your child’s STAAR results, please contact the school and we will assist you in obtaining the information.

Forman's Report Card
The State 2016-2017 School Report Card for Forman has now been released, and posted to the Forman website at https://www.pisd.edu/Page/51. The report provides information concerning student performance on the State of Texas Assessments of Academic Readiness (STAAR), as well as information on student enrollment, class size averages, and financial expenditures. The information contained in the School Report Card is described in the “School Report Card Definitions.” If you have any questions concerning the School Report Cards, please let us know. Its that time of year again when our student start to collect Boxtops for our school.
Help our school earn cash by clipping Box Tops from products and sending them to school! Each one is worth 10¢ and our school can use the cash for whatever it needs. See a list of products at BTFE.com/products.
Parent Portal
Please go into the parent portal http://parentviewer.pisd.edu/ and fill in all required information. This year's portal is now open and is where parents or guardians will update contact information; update student health information; select restrictions; and electronically sign emergency forms and other annually renewable forms.

Voluntary Student Accident Insurance
Student insurance is available for purchase through Markel Insurance Company. Parents can enroll students online using Markel’s secure enrollment website or by
calling 877-444-5014. The website address is http://markel.sevencorners.com/. On this website, parents are able to:
• View all coverage and premium options
• Download and print Claim Forms
• Review a Frequently Asked Questions section
• Purchase the insurance using credit/debit card and print confirmation of purchase
Student accident insurance provides coverage for accident/injury only. It does not cover medical expenses for sickness/illness, preventive care and routine visits. Please see the attached for specific policy exclusions and limitations. Parents are responsible for enrollment and payment of premium directly to Markel Insurance Company.
Due to increasing deductibles and co-insurance requirements, it can be difficult for families to cope with unexpected medical costs. The district provides parents with the option to purchase student accident insurance in the unfortunate event of accidental injury to their child. It is important that parents are aware of this coverage to enable them to review the information and determine its value for their individual family situation. Please share the attached brochures with parents at your location.

Plano Senior High Boys Baseball Summer Camp
Clinic Information
Head Coach Rick Robertson and his staff will lead these baseball clinics for all levels. The coaching staff will include coaches and players drawn from collegiate and high school ranks. The one-week sessions will address pitching, catching, hitting, base running, infield/outfield play, and game situations. Players will be grouped according to age and skill level. Clinic instruction will be conducted in a positive, fun, progressive approach to skill development and personal growth. Come work out on the same field where your high school heroes play. Bad weather contingency will be discussed at first day of clinic.

Online registration through the City of Plano begins April 2 for Plano residents and April 4 for non-Plano residents. No registration is possible until these dates. To register on-line through the City of Plano, go to www.planoparks.org and click on the Registration button. Follow the prompts to either log in OR to set up a new account. Please do not set up an additional family account if you already have one. If you do not have internet access, you may obtain a 2018 Summer Leisure catalog from a Plano recreation center or library beginning April 19. Registration instructions are provided in the catalog.

Register for after-school care at: http://www.pisd.edu/parents/program.pasar/index.shtml If you need care on the first day of school you will want to register as soon as possible. It generally takes a few days to get students into the system. Contact the PASAR Finance Office at 469-752-8915 with any questions you may have.
