2019 Graduation Banner Order Form
Dimensions: All Banners are vinyl, 14" high x 28" wide and have metal grommets.
Costs: Orders submitted by June 1st will be $20 per banner. Orders after June 1st will be $25 per banner. June 12th is the last day to submit an order.
Ordering and Payment: Please complete EITHER the Google form and select an option, or complete this form and return to Mrs. Olsen, Student Council advisor. Payments for Google Form orders must be sent to Mrs. Olsen, Student Council Advisor (Activities Office at the HS) within 2 days of Google form completion. Orders placed with a physical form must have payment attached. Checks can be made to SBHS-Student Council.

*Banners that have a space for "Student's Name", will appear as it is on their school records and the photo used will be the yearbook photo on file. Please do not send a photo.
Delivery: Graduates must pick up their banner on June 24th at cap and gown distribution. Please look for the Student Council Table.
THE GOOGLE FORM WILL BE CLOSED AT 2:30 ON JUNE 12th!!!!!!!
PHYSICAL FORMS WILL NOT BE ACCEPTED AFTER JUNE 12TH!!!!!

Project Graduations May Restaurant of the Month!
May Restaurant of the Month- Franklin Park Diner(next to Urgent Care on Route 27). Bring the attached flier and Project Graduation will receive 15% of sales back!!!
Eat at the diner then come join us for the Project Graduation Meeting at 7:30 pm in the Blue Cafeteria. We are looking for help with bus sign-ups at lunch time May 28-May 31st!
