Student Information Form
Each year all students must have an updated Student Information Form submitted through the parent portal. The information form, accessible through the parent portal, is used to collect items such as family information, emergency contact data, and medical permissions. It is crucial that you complete this form as soon as possible. Failure to submit this form will affect your student’s ability to attend field trips, participate in after-school activities, and receive medications from the school nurse.
Communication with the Main Office

We will use the following email address for ALL communication (i.e. sick calls, tardy calls, dismissal calls, change in child’s typical dismissal process).

Please include your child’s teacher on any email correspondence with the main office. Always indicate child’s first name, last name, and grade level.
Changes to your child’s dismissal process should be communicated as early as possible and no later than 1:00

Don't Forget to Show Your STRIPES on the bus
January is the perfect time to review routines, rules, and expectations. Please take a moment to review this Bus Safety Video with your child. Here is some additional information:
It is recommended that you arrive at your bus stop 5 minutes before your scheduled pickup time.
Be at your stop 5 minutes prior to your child being dropped off. Kindergarten children will not be let off the bus unless someone is there to get them. If there is no one at the child’s stop, the child will be brought back to school and the parent/guardian will be contacted to pick the child up at the school.
Bus warnings will be written up by the driver if your child is not following the established bus rules.
Bus warnings will be sent home with your child to be signed by a parent/guardian. Parents/guardians will be notified by a building administrator if a child receives a bus warning.
Your support in reinforcing positive bus behavior is greatly appreciated. Our hope is that we are able to work together to provide your child with a safe riding experience each and every day.
TES Health Office January 2023 Update
Wellness update- During the last few weeks of December TES received many reports of Influenza. In addition, we are still receiving reports of the common cold, gastrointestinal illness, Covid and conjunctivitis. Kindly review the TPS General Illness Protocols for school attendance. The current Covid guidelines for K-12 students can be found here. Please note that we are no longer conducting Covid testing in the school setting. The Tyngsborough Board of Health has provided the schools with test kits that we can give out at no cost to any family that would like them. Please contact the school nurse via email if you would like free covid testing kits.
If your child tests positive for Covid-19 we are asking that you contact your child’s pediatrician for an excuse note and for return to school dates.
Additionally, please keep the health office notified of any changes in your child’s health status to include medication changes, hospitalizations, fractures, sprains, concussions, etc.
Mandated Health Screenings- Vision, Hearing, and Height/Weight screenings are underway for our 3rd-5th graders. Please keep an eye out for any referral letters that may get sent home with your student. Please keep in mind that these screenings are state mandated and are in no form diagnostic of an underlying health issue. It is our recommendation to contact your child’s pediatrician for guidance if you receive a referral letter.
A notice will be sent out as a reminder prior to 5th grade postural screenings. If your child is screened during their well visit, you may wish to send in a copy of the exam to the Health Office for your child’s records.
If you choose not to have your child participate in state mandated school screenings, a written parental note is required along with proof that your child has been evaluated by a physician for each screening refused.
Please be mindful of the cold weather and dress your students accordingly. Students go outside to play as much as possible. Also please send your student in with an extra change of clothes to keep in their locker in case they need a change during the school day. The health office does not always have clothing that will fit your student and in the event that your child may need an extra set of clothing you may be called.

Mental Health Resources
The William James INTERFACE Referral Service is a mental health resource and referral helpline available to subscribing communities across Massachusetts to access outpatient mental health services. When you contact William James, you will speak with a resource and referral counselor who will complete a confidential intake. Referral counselors will then use the information you provided to match you with providers in our community that accept your insurance as well as match your preferences for appointment times, specialties, and services.
