PLEASE PRINT CLEARLY AND NEATLY
Print your name, as you would like to have it appear on your diploma (formal is best, matching other documents).
_________________________________________________
Print your name, as you would like to have it read at graduation (this is how it will appear in the program also).
_________________________________________________
This is your diploma name card of how you would like to see your name on your diploma and how you want it announced at graduation. (The two may be different, but remember formal is more suitable to the graduation ceremony and your diploma should match your birth certificate for future references.) Please discuss this at home with your parent/guardians and return this completed form as soon as possible to the Counseling Office.
This is your one chance, it cannot be changed after printing, please be very sure of your decision.
