Notes From Nurse G
Dear Holmes Families:
Welcome back and I hope that you have had an enjoyable summer!
If your child will be using medication for the 2019-2020 school year, it is necessary for the District 97 Licensed Prescriber Authorization Form and any Emergency Action Plan (Asthma, Allergy, Seizure) to be renewed. Students with asthma, allergies, or seizures need to have an annual Emergency Action Plan developed and signed by a physician. These forms can be found online at http://www.op97.org/health-services/forms or on my webpage at http://www.op97.org/~aguerrier@op97.org/holmes-nurse.
Please have your child's physician complete the form(s) and bring them to the nurse's office with your child's medication on Supply Drop-Off Night (August 26th from 5:30-6:30pm) or on the first day of school. Medication must be turned in directly to me, medications CANNOT be turned in to teachers or other school staff members.
Please note that per District 97 policy, all medications must be in the original pharmacy labeled container or in a sealed as purchased over-the-counter container. Please check the expiration dates of all medications you turn into the nurse's office: expired medication cannot be given at school.
Please contact me with any questions at 708-524-3098 or aguerrier@op97.org.
In Partnership,
Anne

Snack Procedures at Holmes
- Students will keep their zippered lunch boxes in the class lunch bin outside of the classroom.
- Students may eat a snack of fresh fruit or fresh vegetables in the classroom mid-morning.
- Students will wash hands at the beginning of the day, after snack, and after lunch to help minimize traces of allergens in our classrooms.
- Students should not share food at any time.
No other food items will be permitted in the classroom.
We realize this is different from past years. We are grateful to you and your students for understanding the need for these precautions. Our teachers and entire Holmes staff appreciate your help in keeping our children safe.
