Upcoming Screenings - CHECK BACKPACKS
On Monday, October 16th, the Lion's Club was at our school and performed vision screenings for all of those who consented. Please be on the lookout for results in your student's backpack. If your student does not have results in their backpack, this mean they PASSED. Those who have referrals will have that information in the packet.
Health Partnership Clinic is going to be performing our dental screens this year on November 16th and 17th. Online, you can give consent for or opt out of the screening. I am sending a form home for you to give consent to Fluoride treatment if you wish for you student to have this done. I will only be sending home these forms for students who I received consent for, or if the student does not have any consents entered. If we do not receive the consent form back, a "look and see" approach will be taken. Fluoride will not be given unless we have received consent.
You are able to opt out of any screening that you do not want your child to have. This can be done online or by emailing me.

If you have not filled out online consent forms, your child will be screened. Previous years, we have screened every child unless hearing from a parent that they wanted to opt out. It will be the same this year.
Immunizations/Physicals
Immunizations:
- All students attending Shawnee Mission schools must show proof of having received one of each of the required vaccinations.
- Students have 60 days to receive subsequent doses of required immunizations after starting school.
- Students who are non-compliant with immunizations after 60 days will be excluded from school attendance until written documentation of required immunizations is received.
- Written documentation of vaccinations must be from a medical provider, health department, or school.
- Students who are medically or religiously exempt from immunizations may not be allowed to attend school if there is an outbreak of a vaccine preventable disease for which they are not immunized.
Kansas State Immunization Requirements K.A.R. 28-1-20 defines immunizations required for any individual who attends school or early childhood programs operated by a school.
Immunization Exclusion Date: 10/23/2023
Final notification of vaccination exclusion was sent out 10/16/23. Please check your child's back pack.
Physicals:
Students less than 9 years of age entering a Kansas school for the first time must present a health assessment (physical) in accordance with Kansas Statute 72-5214.
Physical Exclusion Date: 11/17/2023

Allergies vs. Illness
If your student experiences seasonal allergies that requires medication to alleviate symptoms, daily allergy medications must be given at home! We do not have a daily medication to give to students. Recommendations include Children's Zyrtec or Claritin. If you have further questions about which is best for your child, please reach out to their pediatrician.

Asthma and/or Anaphylaxis Action Plans
Medications must be brought in with student identification from the pharmacy in order to be used in the nurse's office. Consent forms need to be signed by parent. This can be provided when dropping off medications, or forms are in the links below.

Clothing Closet
They also have a sock and underwear program where those in need are eligible to receive 5 pairs of socks and 5 pairs of underwear each semester (must show proof of being enrolled in SMSD). If you are in need of the sock/underwear program, please let me know.
