Consent Forms
Please be sure you go on the Pasco Parent Portal and fill out the consent forms. Leaving the consent form blank is a NO for clinic services. If you would like your student to utilize the clinic if they are sick, or get hurt please consent to yes to illness care treatment and RN assessment. If you mark NO or DON'T FILL OUT THE FORMS your child can't be seen in the clinic. Your child will be asked to call you to inform you of the situation. If you do not have internet access at home or a computer, you may come to the school to fill out the consent forms online.
Our annual health screening for 6th grade is October 25th. We screen height and weight, vision, hearing, and scoliosis. If you would like your 6th grader to not be screened, please be sure you indicate that on the consents on the parent portal, there is a screening section on consents. Please do this well in advance to the screening, not the week of.
Transportation Employment Opportunities!!!
Pasco County Schools is currently looking for 52 regular route drivers, 44 relief drivers (who fill in for vacancies or absences) and 19 Transportation Assistants. We have full and part-time opportunities across the county, and if you or someone you know might be interested, we would love to have you join our team. We offer paid training, the opportunity for a great benefit package, and a schedule that works well for parents of school age children. Starting pay begins at $15.00 for Transportation Assistants, $15.38 for Drivers, and $15.76 for Relief Drivers. Our next training class with openings begins on October 31st and November 28th. Students who missed our picture day on September 13th, will have the ability to get their picture taken on October 11th. The qualifications for makeups:
- They were absent or new since picture day (Sept. 13th).
- They purchased a package and are returning their photos for a new photo.
- They planned to purchase but were unsatisfied with their first photos.
Our photography company was given a list of students who were absent/missed picture day.

1st Ever CCMS Talent Show!
Attention Coyotes! Do you consider yourself talented? Are you a good singer, dancer, musician, magician, comedian, artist, or poet? Or maybe you have another interesting miscellaneous talent. Well, get your acts ready because... Cypress Creek Middle School wants to showcase your amazing talents at our 1st ever Talent Show!
If you would like to perform at our talent show, auditions will be in the Black Box theater from 3 pm - 4 pm on October 18th and October 19th. You must audition with the performance you plan to do on the night of our talent show. Acts should be no longer than 3 - 4 minutes in length. The final performer's list will be posted in the Cafeteria on October 24th.
Tickets will be on sale starting October 28th, through Rycor (MyPascoConnect Portal) for $5.00 apiece.
Pick-Up Procedures Reminder
This is just a reminder that all car riders are to be picked up in the afternoon by the covered awning in front of the school. Reports have been made that students are walking to the loop of the Cypress Creek campus, instead of being picked up at the designated location. This poses a serious safety risk. It is the expectation that all parent drivers form two lines and follow the traffic pattern in our parking lot. Our pick-up procedures are smooth and efficient. The wait time is significantly less than what it was at the beginning of the year. Please follow these procedures so we can ensure the safety of our students. Thank you for your cooperation and support!
FAST Results
This year the state eliminated the end-of-year Florida Statewide Assessment (FSA) and has moved to a progress monitoring model called Florida Assessment of Student Thinking (FAST). Under the progress monitoring model, student progress is monitored throughout the school year to provide constant feedback on academic progress.
The state’s first FAST testing window is now open. Your child’s teacher will use the results of this assessment to measure general achievement in reading and/or math and to determine your child’s learning needs.
It is important to understand that these test scores provide only one picture of how your child is doing in school. At the beginning of the year, students are not expected to have mastery of grade-level skills. Students are taught skills throughout the year and should move towards mastery as the school year progresses.
If your child has already taken FAST PM1 (the first progress monitoring assessment), the scores are loaded into the Pasco Parent Portal. Directions for accessing these scores as well as additional information regarding FAST testing and interpreting the scores can be found at the Pasco Schools’ Accountability, Research, and Measurement page devoted to FAST testing.
If you have any questions, please reach out to your student’s teacher.
Healthy Student
Please fill out the 22-23 healthy student application if you would like your student to be a part of the program. Healthy student is a great way to keep students in school. Healthy student includes management of acute illness or injury and the administration of limited medications, following physician guidelines and protocols (i.e. ibuprofen, Tylenol, Motrin, Robitussin, Tums, and antifungal ointment, Benadryl, hydrocortisone, etc.).
Please fill out the attached form and return it to the clinic or email to lgriffen@pasco.k12.fl.us if interested. We CAN NOT take any verbal consents for medication administration so not filling out the form means a call home if your child isn't well with a potential need for pick up.

Schools receive a portion of their funding based off the percentage of students who qualify for free or reduced-price meals. Notice Of Inspection for Asbestos containing materials in Public Schools
The Environmental Protection Agency (EPA) has issued a final rule, 40 CFR Part 763, Asbestos Containing Material in schools, commonly referred to as AHERA. This rule requires all Local Education Agencies (LEA’s) to identify asbestos containing materials (ACM) in their school buildings and to take appropriate actions to control the release of asbestos fibers.
All inspections for Cypress Creek Middle School have been completed and a Management Plan describing the results of the inspection and the action plan to control any asbestos found during the inspection, where applicable, has been submitted to the governor for approval. The Asbestos Management Plan for Cypress Creek Middle states that “no asbestos building materials were used in the construction of the facility”. A copy of this plan is located in the administrative office of this school and at the Pasco County Schools Maintenance & Facility Services Department and is available for review upon request.
