Let's Try It!
Here's a fantastic link- and check out the clip at the bottom that shows this in action. "EL: Core Practices in Action" means Expeditionary Learning, which is founded based on Outward Bound principles.

It takes a Village
Go back to basics, use the three strikes, use sports analogies about fouls, etc. When a student earns three strikes- and this is not necessarily per day, you set the parameters on that, but explain the consequence. Lunch/Recess detentions (with a phone call home, of course) and/or more serious consequences are meant to teach the appropriate behavior. We will support you in every possible way, but it does take a village. We may feel like one lego piece, but put our efforts together and we build greatness!
Returning to assigned seats in the cafeteria looks more like a reality, and we can go to boy/girl seating, too- or perhaps inviting Mom or Dad in to sit with their child to help them understand how we roll. On that note, please review your duty schedule. I completely get that it's tough to get to where you need to be- just put your best effort in and arrive on time. As we all know, the transitions are where the most problems occur. Adult supervision is the most effective way to curb this- in other words "WE NEED YOU".
