WEB
Where Everybody Belongs (WEB) is a middle school orientation and transition program that welcomes 6th graders and makes them feel comfortable throughout the first year of their middle school experience. Built on the belief that students can help students succeed, the program trains mentors from your 8th grade class to be WEB Leaders. As positive role models, WEB Leaders are mentors and student leaders who guide the 6th graders to discover what it takes to be successful during the transition to middle school and help facilitate 6th success.

Immunization Requirements
Incoming 6th graders
Must have documentation for Tdap vaccine within two weeks of turning 11 years old. NYS Law 2164 REQUIRES students entering 6th grade, or a comparable age-level special education program receive a Tdap (tetanus toxoid, diphtheria and pertussis) vaccine prior to entering school.
Incoming 7th graders
All students entering/attending grades 7 are required to receive a meningococcal conjugate vaccine (MenACWY) per NYS Public Health Law 2164. Meningococcal meningitis is a serious disease that can lead to death within hours. Survivors may be left with severe disabilities, including the loss of limbs, cognitive deficits, paralysis, deafness, or seizures.
Physical Exam Requirement
New York State law requires a current physical examination for all students upon entering grades K, 1, 3, 5, 7, 9 and 11. The examination must be completed and signed by a New York State licensed healthcare provider. A Dental Certificate which states your child has been evaluated by a dentist or dental hygienist is also requested at the same time.
The Health Office fax number is 585-216-0060.

- September 1st, 8:00 - 3:00

Dispensing Medications
The District strongly recommends that all medication be administered to students under the supervision of the nurse. If you must take any prescription or over-the-counter medication during school hours and/or at school-related events, State Law requires the following:
1. A written request from your health care provider
2. A written request from your parent/guardian
3. Medication in a pharmacy- labeled container for prescription medications and original manufacturer’s container for over-the counter medications
4. Medication delivered to the nurse by an adult, unless required documentation for Independent Use and Carry of Medication is on file and approved by the nurse
*You may not store medication in your locker nor carry and/or take medication in school or at school-sponsored events unless the required documentation is submitted to the Health Office. This includes prescription and over-the counter medication, dietary pills, supplements and vitamins. If you carry or use unauthorized medication or share medication with other students, you will be subject to disciplinary action as outlined in the District’s Alcohol and Other Drugs Policy, including suspension from school and referral to the CAP program.
