A note from our Nurse!
We are in the swing of the 2017-2018 school year. I hope that you and your student have settled in and are having a good year. Here are a few notes from the Nurse.
1. If your student has a medical condition or something develops during the school year and you aren’t sure if I am aware, please contact me by phone (317-838-3950) or by email (swallon@plainfield.k12.in.us). Plainfield Middle School does our best to accommodate everyone.
2. I will contact a parent to get permission each time I give a student medication from our school supply (ex: Acetaminophen or Ibuprofen). If you prefer, you may provide that medication in the original container with a note from a parent. Your child may then come for the medication whenever they wish. You would not receive notification that they are taking it.
3. The required immunizations for incoming 6th graders are the Tdap (Tetanus & Pertussis) and the MCV4 (Meningococcal). When your student gets ANY new vaccines, please send documentation in so that the records can be updated. All immunization records must be up to date by September 29, 2017, to avoid being excluded from school.
4. Please notify the guidance secretary any time that contact numbers change. We need accurate phone numbers to reach you in case of an emergency.
