Annual Notice of MAP Test and Testing Windows
Every year District 15 students in grades 2 to 8 take an academic test called the Measures of Academic Progress (MAP). The MAP test measures each student’s achievement in reading and math. Most students complete the MAP in less than 40 minutes. This test provides our staff with valuable information that helps plan instruction and track student growth. Students take the MAP once in August or September and once again in April or May. Sundling students will take the Reading and Math tests during the week of August 30th (Reading- 8/30 & 8/31 and Math- 9/1 & 9/2).
Please visit our District Assessment Calendar on the District 15 website if you would like more information about the MAP test. It is always helpful to make sure your child is well rested and prepared for MAP testing. If you would like to have your child practice the MAP test, you can visit the MAP practice site at: http://bit.ly/PracticeMAPTest.
MAP testing is completed at school. However, if you have a student that qualifies for virtual learning through a medical exemption, your child will take the MAP test remotely (at home) at the same time as their classroom peers.
The MAP test helps school staff determine our students’ reading and math skill levels and helps our schools pinpoint specific areas to focus on during instruction. School staff also use the MAP to monitor each child’s academic growth. MAP test results from the fall and spring testing periods will be available to parents via the Infinite Campus Parent Portal. These results are typically posted in October and again in June.
If you are interested in more information about the MAP test, please contact me, your child’s teacher, or download the MAP Parent Toolkit at https://www.nwea.org/parent-toolkit/

Guiding Documents and Information
IDPH All Sports Policy; IDPH All Sports Policy FAQ’s
CCSD15 COVID Protocols, Mitigation, and Contact Tracing procedures apply to all Interscholastic Sports teams.
Participation Requirements for All Students
- Completion of Registration,
- Current Physical Examination on File PRIOR to Participation,
- Non-Refundable $85 Participation Fee,
- Agreement to follow all COVID mitigation measures as required per sport.
Highly Recommended for All Students
Per the IDPH update as of August 10, 2021, “To facilitate safer participation in sports, the CDC recommends sports organizers implement screening testing for participants who are not fully vaccinated. Screening testing can be critical to identifying asymptomatic cases needed to interrupt SARS-CoV-2 transmission.”
- CCSD15 SHIELD Screening Enrollment - This screening allows for early identification of pre-symptomatic and asymptomatic cases so that the rate of exposure can be contained to the greatest extent possible. We highly recommend that all athletes enroll in the program as a way to minimize exposure and keep our students safe and competing. Opt-in for SHIELD testing by signing this consent form and emailing it to ccsd15@ccsd15.net.
- Proof of Vaccination - If your student is vaccinated, you can bring their card to your school’s front office to be copied. Providing proof of vaccination enables your student to remain eligible to participate in the event of a positive COVID-19 exposure on your team as fully vaccinated persons who remain asymptomatic are excluded from quarantine.
Spectator Expectations & Information
Per the IDPH update as of August 10, 2021, “All spectators, regardless of vaccination status, should wear a mask in attendance at indoor youth sports events and must wear a mask if the event is held indoors in a public or nonpublic P-12 school setting. For outdoor youth sports events, spectators who are not fully vaccinated should wear a mask when unable to maintain recommended physical distance of at least 6 feet from non-household members or when the event is held in a community with substantial to high transmission. Spectators who are fully vaccinated may attend outdoor youth sports events without wearing a mask, though they may choose to do so in crowded settings or if they or someone in their household is immunocompromised.”
Parents, Families, & Friends can play a big part in keeping our sports programs going by:
- Report Covid symptoms or a positive test to the school nurse as soon as possible,
- Enroll your student athlete in the SHIELD testing program,
- Provide your student athlete’s proof of vaccination,
- Wear a mask to ALL indoor sport events,
- Wear a mask to outdoor events if you are not vaccinated or want to take the extra step to keep yourself and others safe,
- Keep 6 feet of distance while in the stands at both indoor and outdoor events,
- Promote a positive message of encouragement and expectation to your athlete regarding all mitigation efforts. They will follow your lead!
