School immunization records may be found in your Aeries Parent Portal under the Medical tab then click Immunizations.
Please call your child’s doctor to make an appointment as soon as possible. If your child does not have health insurance, please contact the Orange County Health Care Agency at (800) 914-4887 for immunization assistance.
Immunization records may be submitted to the school health office or uploaded to the school nurse here. Your child’s immunization record must contain the date the vaccine was given and the doctor’s information, including an office stamp.

Socio-Emotional Learning (SEL) Student Survey
In an effort to ensure that all of our students are thriving in Tustin Unified School District, we are replacing the Gallup poll administered in previous years with a Social-Emotional Learning survey. District-wide, students in grades 4-12 will take the survey twice during the school year, on September 30th for a baseline snapshot, and between March 1 and March 12 as an assessment of school-wide progress.
The purpose of the survey is to gather information regarding general trends in order for school and district teams to plan how they can better support students’ social and emotional development. Students responding to the survey will not be asked to give their name, and survey results will not be disaggregated by individual students for analysis.
If you would NOT like your child to participate in the survey, please EMAIL Mrs. Bojorquez at hbojorquez@tustin.k12.ca.us or call the office at 714-730-7573 by September 29, 2020.
If you give your consent for your child to participate in the survey, no action is required.
