Food Service Selection Form

Dear District 112 Parents and Guardians,
On April 6 (K-5) and April 12 (6-8) District 112 students returned to full in-person learning. In order to further support this transition, District 112 offers prepackaged lunches and breakfasts free to all students, at all K-8 schools on days when school is in attendance. Meals are provided by OrganicLife, which is our district contracted food service provider.
These meals are funded through a Federal program at no additional cost to the district or families. Students who are attending in-person at any K-8 school are given the option to opt-in to the program.

Parents must complete this form by Monday, April 20, 2021.
If you submitted the first food service selection form sent on March 19, and now want to change the option you selected, fill out this new form. If you filled out the previous form and you want to keep that selection, you do not need to fill out this new form.
Lunch will be prepackaged meals, available for students to take as they go through socially distanced lines in the cafeteria.
Specific meal offerings may differ from building to building, depending on each building’s kitchen facilities and cafeteria capacity.
Breakfast will be “grab and go” style, with students taking prepackaged meals home at the end of each day to be consumed the following morning.
All meals will conform to USDA nutritional guidelines as required by the Seamless Summer Program.
