Health Insurance Open Enrollment
If you have received notification from your PCP stating they will no longer be accepting your current health insurance plan, this enrollment period is your opportunity to research your options.
Step 1: Review the Changes
While the name will change, everything else will continue – same doctors, same hospitals, and same coverage. There will be no impact or change to your access to healthcare providers, or to your level of support as a result of any brand changes. Your health plan will continue to operate as it currently does. Wellpoint will begin mailing new member ID cards in June of 2024, prior to the start of the new plan year. Until then, please continue to use your existing Unicare ID card to access care and services.
Here are the new plan names:
- Unicare Total Choice Basic (IND) will become Wellpoint Total Choice (IND) | New England and International Residents
- Unicare Indemnity Community Choice (PPO) will become Wellpoint Indemnity Community Choice (PPO)
- Unicare Indemnity Plus (PPO) will become Wellpoint Indemnity Plus (PPO)

Option 1 and Preferred Method: MyGIC Portal
The GIC portal is available to all subscribers to keep you up to date on information, and to make any changes online. By utilizing this Member Benefits Portal, members also ensure their preferred email address will be added to the GIC database so you will receive all future important electronic communications from the GIC. - Search for Health Insurance Form
- Complete the form
- Attach the required documentation (if applicable)
- Save Final and Sign
Attention Please!
- If you are enrolling your spouse, you will need a marriage certificate. If you are enrolling dependents, you will need their birth certificates.
- If you are enrolling any dependents over the age of 19, please be sure to complete the Dependent Age 19-23 Enrollment/Change Form.
