Self-Certification
If your child has been a remote learner and is now returning in-person, you need to download and print the self-certification form that is linked below. Your student will receive a packet of self-certification forms on their first day back on April 7th to use on subsequent days , but will need the downloaded version ahead of time for entry into the building on the first day.
Please click here to access District 140’s Self-Certification form from ksd140.org. This form must be filled out and signed daily. It will be collected by the first point of District contact, (i.e. bus driver, teacher, or paraeducator) at the bus stop or entrance to the school.
In order to keep our doors open, it is imperative that families answer the daily self-certification questions honestly. Please keep your child home if they have a fever or are experiencing symptoms. By working together, we can keep everyone safe and in school.

5Essentials Survey
Please remember, the 5Essentials survey closes April 2nd. Your opinion is important to us.
Please click here to take the parent survey.
These surveys will serve as an opportunity for schools to reflect on progress since the last statewide implementation. School and District leaders use their 5Essentials data to better inform their school improvement plans and help identify key strengths and areas for growth. Your identity and survey responses will be kept completely confidential and will never be connected to you or your child.
