Welcome to the 2021-2022 school year, LinK12 families!
Our enrollment growth afforded us the opportunity hire some great new teachers for the upcoming school year.

Please note: We have a LinK12 planner for every middle and high school student. You may pick it up on September 2 or 7 from 8:00am-2:00pm. All 6th graders and new to LinK12 7-12th graders will receive their planner at orientation on September 1. More details are below.

What is the Parent Portal? Infinite Campus is available for students and parents/guardians. As a supporter of your child’s education, it is important that you have your own Parent Portal account so you can better understand, monitor and participate in the educational process with real-time access to your student’s information, including:
Assignments
Attendance
Report Cards
Schedules
The Parent Portal is also where you can find the latest district and school announcements and select your preferences for receiving email communications from your child’s school.
How do I get an account? Infinite Campus Parent Portal accounts are set up for parents/guardians when you enroll in any school in the district.
Stop by the Lakeville School District Office with your photo identification on September 2 from 8:00 a.m.-2:00 p.m. or September 7 from 8:00 a.m.-2:00 p.m. Contact Amy Bercich at 952-232-2023 if you have any questions.
You can also send a picture, scan or fax of your photo ID to sissupport@isd194.org, and instructions will be emailed back to you

I forgot my username or password:
Usernames and passwords can be set or changed using the Forgot Username or Forgot Password option at the Infinite Campus Parent Portal login page, without needing to contact support. If you are still having difficulty logging in, please request assistance by emailing SISsupport@isd194.org.

Immunization Information
Minnesota law (https://www.revisor.mn.gov/statutes/cite/121A.15 ) requires children enrolled in school to be immunized against certain diseases or file a legal medical or conscientious exemption (Immunization Rule 4604.0400) . Certain immunizations are required to be administered before the first day of school or documentation of a medical or conscientious exemption when entering 7th grade.
If you are declining immunizations for personal beliefs an updated notarized Conscientious Exemption Form or Legal Medical Exemption Form must be submitted: Immunization Record Form - MN Dept of Health
Due to limitations brought on by COVID 19, it is very important that you consult with your medical provider as soon as possible to schedule well-child checks and immunizations. Immunizations are deemed essential medical appointments to minimize serious health impacts for your child and ensure the safety of others in the Lakeville community. The MN Department of Health cautions that slowing or stopping access to vaccinations increases our risk of other disease outbreaks.
If your child has updated immunizations please email records to enroll@isd194.org
Remember Dakota County offers reduced cost immunizations for those with financial need. For updated information on the Dakota County Public Health Clinic schedule, please call them at 952-891-7999.
Your child must have a copy of their updated immunization record or a notarized Conscientious Exemption form or legal medical exemption form on file before the first day of school this Fall.

Annual Household Check-In

Each year, Lakeville Area Schools asks families to complete paperwork to review, confirm and update their household and student information for back-to-school. We are pleased to move this process online for the 2021-22 school year.
Families will be asked to verify contact information, communication preferences, and student health information. Families will also be asked to review and agree to the Acceptable Use Policy for student devices, the iPad loan agreement and their school’s student handbook.
This check-in will open for the 2021-22 school year on August 1, and close on Sept. 1, 2021.
Please verify your child(ren)’s information through the Infinite Campus Parent Portal. Below are the steps to complete the Annual Household Check-In process:
Step 1: Log into your Infinite Campus Parent Portal account - Please review the information below if you have questions about how to sign in to the Parent Portal.
Step 2: Select MORE from the menu along the left side of the screen
Step 3: Select Annual Household Check In
Step 4: Select Begin Registration

Important information to know before you get started:
Only one parent/guardian per household will need to complete the Annual Household Check-In process.
The Annual Household Check In link is only active for one household per child. If your child has more than one household, the link will be active for the parent/guardian that lives in the household that is currently indicated as the child’s primary household.
