IEP- Secondary Transition Present Education Levels/Assessments
Did you know?
Secondary transition is the process of preparing students for life after they leave high school, including participation in post-secondary education or training, employment, and community living. The entire process is based on individual student’s needs, taking into account each student’s strengths, preferences, and interests.
To help facilitate a successful transition from high school, students should be given assessments, interest inventories, surveys, checklists, questionnaires, rating scales, person centered planning tools, and or preference and aptitude tests yearly to identify and support the students goals after high school. This data guides the activities the student will participate in or have exposure to help ensure the bridge to adult life is smooth.
