News from Carl Sandburg Junior High

A Note from Mrs. Johansen
Dear Sandburg Community,
It's hard to believe the 21-22 school year has come to a close. I'm so proud of all we accomplished with our return to in-person instruction, reinstating student clubs, participating in inter-scholastic sports, and the long-awaited return to everyone eating in the cafeteria! We were able to do all of this while adhering to protocols to keep our kids and staff safe! In addition, what an amazing celebration last week's graduation was!
Included in this newsletter are some important pieces of information as we head into the summer break.
As you know, the 4th quarter concluded on Friday. Your student's report card is now viewable on Infinite Campus. If you are having any challenges
We have 15 extra yearbooks in the office, if you would like to purchase one for your child. They are $12 each. Please call the Sandburg office 847-963-7800.
The next communication from my office will be after the 4th of July holiday weekend. Our communication data bases should be updated at that point, which means if you're not attending Sandburg next year, you will not receive future emails from us.
To our 8th grade families, I wish you all the best as you begin your transition to high school. I've enjoyed getting to know you and will miss you next year. Please keep in touch!
I continue to be grateful to be apart of this school community. Thank you for your continuous support!
Have a wonderful summer!
Erika Johansen

SAFE2HELP Illinois
In the absence of a trusted adult, Safe2Help Illinois offers students a safe, confidential way in which to share information that might help prevent suicides, bullying, school violence or other threats to school safety. This program is not intended to suspend, expel, or punish students. Rather, the goal is to get students to 'Seek Help Before Harm.'”
"Safe2Help Illinois is a 24/7 program where students can use a free app, text/phone, website (Safe2HelpIllinois.com) and other social media platforms to report school safety issues in a confidential environment. Once vetted, the tips will immediately be shared with local school officials, mental health professionals and/or local law enforcement, depending on the nature of the tip. The program also will help local officials by connecting them with mental health resources or other appropriate resources to intervene and help students before they harm themselves or others."

From the Sandburg Nurse
Hello!
Happy Summer! Here are a few notes as you head into summer break!
- Annual health examinations are required for interscholastic sports participation. Physical exams are required- before trying out- for participation in an interscholastic sport. The exam form, completed and signed by a doctor, must be dated within 13 months prior to the start of practice or tryouts, and on file in the school nurse's office. Physical forms are available in the school office, online, and here: Certificate of Child Health Examination.
- All students new to District 15 will require a current physical exam with all immunizations up to date (Certificate of Child Health Examination), and students new to the State of Illinois will require an eye examination. This form is available in the school office, online, and here: State of Illinois Eye Examination Report.
- All students must provide proof of immunizations by October 15, 2022, or the student will be excluded from school. The list of current immunizations can be found on the district website here: IDPH Health Requirements.
- Students with health conditions such as asthma, epilepsy/seizures, severe allergies, and diabetes require the emergency action plan completed by their physician along with the Medication Administration Forms, signed by the physician and parent, for any medications required at school. Please bring all forms and medications on or before the first day of school.
If you have any questions regarding the above requirements, you may reach me at annerink@ccsd15.net. I will be back in the office the first week of August to answer any phone calls.
Stay safe and enjoy the rest of your summer!

Reminder- Don't Forget to Register for the 2022-2023 School Year
Online student registration for the 2022-23 school year is now open. If you have not already started this process for your student(s), please visit our registration page to start this process.
Registration is the first step in getting ready for the up-coming school year. After you register your child online you need to:
Pay student fees. Our student fee webpage outlines the District’s student fee structure and the three ways fees can be paid.
If you have a student that is starting Kindergarten, entering fourth grade, or entering 7th grade, you are required to prove residency in District 15. Our residency webpage outlines the documents you will need to provide to prove residency.
All three steps of the enrollment process must be completed prior to the start of the school year. Students of families that, when required, do not prove residency, will not be able to attend class in a District 15 school until the family’s residency is verified.
