School Forms to be Completed

Our annual Beginning of Year Packet has been mailed and many of you have already received it. If you have not received yours by Tuesday, August 3rd, contact the office. We will need to verify your address.

Please complete the forms enclosed in the packet and return them during orientation or on the first day of school. Please do not email them.

In addition to the forms in your packet, please visit the JCPS "Parent Place" to complete:

  • Nurse Office Consent
  • Home Language Survey

Afternoon Transportation

Parents were mailed letters this week that indicate how we show your student WILL BE GOING HOME. The letter does not say anything about the mornings. If you have a bus stop in the afternoon ... you have the same stop in the morning unless you have told us otherwise. You do not need to contact the office to confirm. The purpose of these letters is to help ensure your child is sent home via the correct route. (If you did not receive a letter, we were still setting up your bus stop. At this time, everyone who requested a bus and is eligible for one has a bus stop created. You can find that stop on JCPS Bus Finder.)

Bus riders are given a backpack bus tag at orientation. This tag tells the bus driver exactly where your student will be getting off of the bus. Check the tag carefully for accuracy. It must remain on your child's backpack for at least the first two weeks of school.

Use the JCPS Bus Finder to determine the stop that serves your address and what time your bus will arrive in the mornings.

Medicine Distribution at School

Staff are not permitted to administer any medication without signed documentation from a physician. This includes over the counter medications such as Tylenol, Ibuprofen, Cough Drops, Sun Screen, Lotions, etc... Please contact if you need a form for your physician to complete. All medications and forms are to be given to Teresa in the Stopher office.

NOTE: Students are not permitted to carry medication (including cough drops). Inhalers may be carried by a student provided there is a form on file with a physician's permission to do so.
